Hi,

I've never seen that either.  There's no real need to press the m's quickly.  
The system should toggle between the two menus each press of the m key.  So, 
VO-m once should take you to the regular menus, keep your VO keys down and 
press the m a second time and it should take you to the Extras menu.  
Continuing with that pattern, each press of the m will take you to the other 
menu set.

Dave, do you get any alert noise, or bonk, or bing or anything of the sort when 
no Extras appear?  Know that the Extras menus are navigated differently than 
the regular menus.  That is, you need the VO keys depressed at all times to 
efficiently navigate within the Extras menus, whereas that is not necessary for 
the regular menus.
