Hi Dave,

Try using control-option lock.  Hold down the VO keys and press the semi-colon. 
 This locks down the VO keys.  Now press m a number of times to see if it 
toggles between the two menu sets.  If it does, then it's likely something to 
do with either your control or option key not being pressed, or losing its 
press action for some reason.  If you still don't get the Extras menu when the 
control-option lock is on, then we have more investigating to do.  You turn off 
the control-option lock by pressing the semi-colon while the lock is enabled.

The problem is poorly stated. I should not have said disappearing Extras Menu, 
I should have said “non-appearing” Extras Menu.
