When you have "delete mail on server" unchecked in the prefs, MD
creates an "UIDL.history" file in its Data directory where it stores
the UIDL's of each downloaded message. Before downloading it just
checks those UIDL's against the ones of the messages on the server and
only downloads those that aren't in the history.
All versions past 1.2 however have the bug that they only add a couple
of emtpy lines to the UIDL.history for every mail and so it doesn't
work anymore and it always downloads all messages on the server.

The question is if it'll be before my patience ends and I look into
using another mail/newsreader (I want them both in one, thats the only
problem, but *IX should offer enough of them).
As Marion said, MD is the Vapor product that seems to have gotten the
least attention from him the last months.
Just found another bug, wanted to drag the Precedence: header form the
shown headers list to the storage one in the Prefs, which resulted in
all headers below it being removed from the list, some moved over to
storage, some got lost completely.
But thats really a minor one given the lost references on edit (and
anyway incomplete references), messed up message sorting of fake
thread heads, UIDL etc. issues...
