The outcome here depends on the threads, in other words did the bolt just
gradually back out or are the threads damaged?

Best case: you get a new bolt, and you live happily ever after.

If the threads are damaged, in theory you can try to machine the crank but
no one I have talked to supports that on a diesel, in practice.

The alternative is a new crank.

I don't enjoy telling you this, but it's based on my own experience a few
years ago. I was lucky... the bolt went in, tightened okay and everyone
breathed a sigh of relief... when it was still there a couple of months
later.

Was your engine rebuilt at some point?
