The slip clutches can wear out and fail; had it happen on a 91 corolla; back in 
2000, the motor alone was $300.

With the w210 (canbus versions), the rear motors are "smart" insofar as they 
have the door control module integrated with them. The fronts are separate from 
the door control module. The only "programming" required is to run the window 
to the limit and hold the switch for a few seconds (not all the way down into 
"express"; just the normal rolldown). Not doing this only disables the 
"express" up/down feature on that window. (you have to do the same thing when 
reconnecting the battery and it is documented in the OM.

I believe the mechanism used for the limit is a magnet on the motor shaft, a 
hall effect sensor, and a revolution-counter...
