I had a similar problem with my mon.cgi server. I did everything I could think of including updating Apache and mon.cgi. Ultimately, it turned out to be a simple problem. My server's date was an hour behind my desktop. As a result, the cookie that was set by mon.cgi expired immediately.
So, check the time and date on your server and desktops.
