Hi David,

  I was the technical editor for _PHP4 A Beginner's Guide_ by McGraw-Hill, which 
covers PHP4/MySQL and written by William McCarty a professor of IT at Azusa Pacific 
University.  He is also a contributing editor for Linux Magazine.

  This book is completely aimed at the beginner, and the code is available online.  I 
tested it on both win9x and Linux/FreeBSD so I know the code works :)

  You can find links to the book, bio, etc. at the site in my sig below.  I'd normally 
not mention it, but it wasn't on your list.  It covers everything from e-commerce, 
intranet, network management, to database administration.
