Hi David, I was the technical editor for _PHP4 A Beginner's Guide_ by McGraw-Hill, which covers PHP4/MySQL and written by William McCarty a professor of IT at Azusa Pacific University. He is also a contributing editor for Linux Magazine.
This book is completely aimed at the beginner, and the code is available online. I tested it on both win9x and Linux/FreeBSD so I know the code works :) You can find links to the book, bio, etc. at the site in my sig below. I'd normally not mention it, but it wasn't on your list.
