Expanding on what Matt said, there are some applications or combinations
of circumstances in which <Ctrl>v works, and others where only the
middle button works.  In KDE, with Klipper running, I can use <Ctrl>v in
many applications, but in console only the middle button works, and I
usually have to have the (mouse) cursor very close to where I want the
paste to occur.
