It's  hard to say. In the US  insurance companies are marginally more  
competitive than say doctors or nurses since most people get to choose  
between 2 or 3 and they quote rates. On the other hand,  some  
physicians try to convince their patients that under 'socialized  
medicine' or even under HMOs they get not personal choice in their  
physician.
