Ron, this is a good example of where specialties, training, and 
practice come in.  I am not sure how I would approach the handshake 
thing... but when it comes to pain, I have been working in a pain 
management clinic for the past four years and have been studying pain 
management and attending continuing ed classes on the subject.  I 
would take that patient in a heartbeat!  Pain is not something that 
was covered, or even addressed, in school.  It's something that I 
learned in the course of my employment.  I feel that I would be 
better suited than most OTs and/or PTs to treat this particular client.
