Dear Liz,
you should mention to them that there has been no good studies into vbac in a birth centre to be able to make a true decision about the safety of vbac.  There is one study that compared vbac in labour wards and vbac in birth centres and shock horror there was no ruptures in the birth centres and the vbac success rate was in the high 90's.  There were no ruptures in the labour ward either but the rates were lower.
The study they were referring to with mutter mutters, was the multi centred study into vbac and it only looked at vbacs in the labour wards.  It did say that there was other options of care that had higher rates of vbac success but they did not feel the need to address these!  Convenient huh?!
Look, there is a great deal of fear and misinformation about vbac in the birthing world of professionals.  This is only increased when there is an adverse outcome due to mismanagement of the vbac labour.  It was not the fact that they induce/augment labours, or push vbac women to preform to a set schedule that really is questionable that causes ruptures...of course not. (for clarity...sarcasms dripping from my chin at this point).
I am questioning whether the obs are in fact making more likely with the new techniques of suturing single layer in the initial cs??  Will vbac be a thing of the past like breech vaginal birth simply because of time saving measures or quick fix jobs by professionals?
Hmmmm...
Thanks again for trying with the petition Liz.  Perhaps you would like to come to a CARES meeting one day?
The next day time meeting is this coming Wednesday at my house in Nairne.  That invitation goes out to all SA ozmid listers by the way.  Give me a call if you are interested.
cheers

Thanks Sandra, some good suggestions there. I definitely agree with the idea of speaking of it at every opportunity. On another note, I brought out the petition that Jo sent me about vbacs in the birthing centres while on placement on an antenatal ward the other day, and none of the midwives would sign it! Something about "mutter, mutter, latest research, mutter, mutter, increased risk, mutter, mutter, continuous monitoring, etc...I told them that studies need to be read critically, that the increased risk of uterine rupture in the study I just read (2001) was tiny, and that regardless, women need to be able to decide what it means for them, and still have that choice available. No response. *sigh*
Liz.
