Been there done that!  I worked for 20+ years the same way
you describe but have other plans for the future either move
or retire to the country in about 5 years. Always
felt "tied" to the clients and work and that will never fullfill
my dream of retiring someday outside the city, travel for more
than 2 weeks, etc.

What I did was to find a much younger friend that I trained, he
attends the clients so he gets the maintenance fee entirely I get
a percent on new sales and the updates fee.  He is a computer tech
so he gets sales on equipment too.  They don't ask anymore
when I will retire, about my age nor if something happens to me.

It wasn't a pavement of roses, at first was difficult to send him
alone since they always wanted to see me, but I accompany him for
about six months.  I sent a letter to all my clients telling them
that he was in charge of maintenance from now on and I will remain
on the back burner, but they always call me first and I had
to politely remind them to call him.  The calls eventually were less and
less...
now after 6 years my phone doesn't ring constantly anymore, in fact
they don't call me anymore so I can concentrate on programming
and medical billing that are my two passions.
