I believe I've found and corrected the cause of the problem. Over the next week I'm going to be testing some various scenarios to make sure nothing else has broken.
chaug - would you be comfortable testing this when I am done? It would involve making a backup copy of a Plugin file on your server, copying a new file in its place, restarting the server, and testing your scenario. All things considered, I'm a little reluctant to release this publicly into the server plugin auto-update mechanism after 10 years of no changes.
