I've had nothing but good experiences with Album Cover Art Downloader 1.6.0: http://louhi.kempele.fi/~skyostil/projects/albumart/albumart-1_6_0.blog/ It's free, and it works about 80% of the time.
In those instances where Album Cover Art Downloader can't find the art, the secret is to dig down to the largest "detail" view image of the album cover on Amazon. Which will bring up a window like this: http://tinyurl.com/86ttq These images are pretty uniform in size (usually around 300x300 which is big enough for most applications) and these images never have the "20% off" etc. overlays. If you are on a PC, just mouse over the image, right click and save the image as "folder.jpg". Done. Irfanview is a great little free image editor, if you should need to crop or scale an image down. Availble here: http://www.irfanview.com/ Regarding cached covers.... I believe this is by design. Rescanning only adds =new= titles to your playlist. If a title already exists in the database, it ignores it. If you want to refresh an existing item, you need to check "Clear library before scan" (same goes for updating tags, etc.). y.
