A follow up on the original question:

It appears that the problem is sox which doesn't properly read what I
believe is a valid 20-bit wav. (I've been through every byte in the wav
file's header and it seems to conform to the Microsoft standard.)

For the time being, I'm going to write a perl script to convert the
file from 20-bits to 24-bits, but I intend to take this up with the sox
developers.
