How is it that you can't tell when a new cover.jpg file has been created in a directory?
Just from playing around with it a bit in Mp3tag, it looks like it only downloads covers and creates either cover.jpg or folder.jpg files (there's an option to set the filename). If you then wanted to embed a cover, it's done in one of several ways - using an 'Import cover from file' action, using the Tag Panel (and Save), or from the Extended tags dialog. You can tell if the cover is embedded in the extended tags dialog, or in the Tag Panel (when you choose in the options to display covers in the Tag Panel. Mp3tag displays the MIME type when it's embedded, usually 'image/jpeg', and displays the filename when it's a separate file. You could also create a new column that displays whether or not there's an embedded cover in a file.
