I haven't had a chance to go look through the old stuff, but I wanna say that the permissions error is a distraction - from what I remember that was really a catch-all error and it threw me off course for a while.
I'd be inclined to say that the sandbox isn't setup properly, and I had a hell of a time figuring that out. I know I had my test users and test merchants set up incorrectly, and once I got that going, it worked itself out.
