That is pretty much the policy that I would like to follow here at
Alaska Clinic, LLC. Has any

US health care group made similar declarations?

As an aside as I was loading the above page I really didn't want to see
all the ads, (among
many others I have seen today on places like news.com and so on) plus we
are on a really slow
DSL line out here in our test labs (on the edge of a 100k acre Moose
preserve) - the solution->
On one of the boxes with a larger disk: 'apt-get install squid adzapper'
(using Debian) then just
added the line 'redirect_program /usr/bin/adzapper.wrapper' to
/etc/squid/squid.conf and
set the proper acl directive, followed 'by /etc/init.d/squid restart'
and pointed the workstations and
laptops to the server on port 3128. Now no more ads, much easier to
read web pages, plus the web browsing is far more responsive/zippy.

Re: [Hardhats-members] Searching VistA documentation

2005-06-18 Thread Ismet Kursunoglu, MD

Jim, David, Nancy and Bhaskar,

Thanks for bringing me up to date with 21 century technology - these
solutions are very helpful.

I still can't see beyond the few lines that the Google results present
without downloading the whole document or HTML translation. For example

using this in a Google search session

site:http://www.va.gov/vdl/ zsystem

yields,

[DOC] BCMA V. 2.0 and This Guide
File Format: Microsoft Word 2000 - View as HTML
CLOSE EXECUTE: U IO K IO(1,IO) C IO ZSYSTEM "lpr -r "_IO. Network
printer:. From the Linux menu:. select System Settings then Printing ...
www.va.gov/vdl/VistA_Lib/Clinical/Pharm-Bar_Code_Med_Admin_(BCMA)/
BCMA_Backup_System_Installation_Guide.doc - Similar pages

Is there a way to extend the context of the search results? I don't see
that as part of the extended or advanced features at Google search. I
also tried alltheweb.com but that didn't support his feature, nor on Yahoo.

Finally What tool would you recommend to create a fast lookup index of
all the documents (hopefully one that is part of the Debian
distribution), including the PDF files i.e. on a locally created mirror
of the vdl? For that matter how does Google accomplish this? I was
looking at Glimpse but apparently it won't index PDF files. Could I use
Fileman or some utility from VistA to do this?

Thanks.

Ismet

You simply have to restrict to a specific site.

For Google, put "site:www.va.gov/vdl" in the search box with your term.
Search term:
fileman site:www.va.gov/vdl

http://www.google.com/search?as_q=fileman&as_sitesearch=www.va.gov%2Fvdl

For MSN Search, put "site:www.va.gov" in the search box with your term.
(Msn and Yahoo don't like folders, only specify the domain)
Search term:
fileman site:www.va.gov

http://search.msn.com/results.aspx?q=fileman+site%3Awww.va.gov

And most of your other search engines will do the same. (Note: Yahoo
will take the expression in the search box but the Advanced Search
"builder" won't place it there for you to easily see how it works. The
MSN Search Builder adds it in while you build out your query so you can
mess with the query while building it.)

/David.

I have also been searching this mailing list archive

Do such search-able sites exist for all the VistA documentation and/or

The Health and Human Services Department released requests for proposals
late yesterday and today that will pave the way for developing a
national health information network infrastructure that will support the
exchange of electronic health records.

The Office of the National Coordinator for Health IT, which resides in

for three of the RFPs.

Since no national health network design exists, industry and regional
providers are innovating, said David Brailer, the national coordinator

data. "That's why we've got to hurry," Brailer said. "The plan that

I arbitrarily created the hfs.dat file having seen this mentioned in

How do I use this device ? Is it purely a method to write out files?
Can I access it over the network? And how does this device relate to
the Linux file system? (in this case a reiserfs) Perhaps someone could
kindly point me to some document that talks about this.

[Hardhats-members] GNU/Linux on laptops-support,installation

2005-06-13 Thread Ismet Kursunoglu, MD
There have been some postings regarding this issue so I hope these
comments are helpful.

I have been installing GNU/Linux, FreeBSD, NetBSD and OpenBSD on laptops
since 1998 with few problems. And now with all the increased driver
support (rare exceptions with some things like wireless chipsets-i.e.
Broadcom-but even there you have ndiswrapper) it is pretty easy to
maintain a reliable system.

The system/package management/facilities, portability, stability and
support provided by Debian make it our "standard" environment of choice.
(In our case we consider "support" being that offered by the extensive
global effort that includes bug tracking/reporting/resolution, automated
security updates/patches, extensive mailing lists, seamless package
management, and reliable system upgrade procedures)

All this along with the various supported platforms are easily available
at www.debian.org

There are commercial vendors all over who will provide
contract/installation/support options of all kinds if need be.
http://www.debian.org/consultants/ And the big vendors are there as
well, for example HP
http://h71028.www7.hp.com/enterprise/cache/76815-0-0-225-121.aspx

And of course there is Ian Murdock's company http://www.progeny.com/ (he
was the founder of the whole Debian project)

(BTW there is a map of "known" users at
http://maps.bzzt.net/users-big.jpg?481,96 and you can register you site
at http://www.linuks.mine.nu/debian-worldmap/)

The latest installer CD makes it really easy to install on most standard
laptops (I am writing this from a Gateway 7405GX AMD 64 laptop) Just get
the right one for your platform. I like the network based installation
ISOs

http://www.debian.org/CD/netinst/

But you have many other choices.

Sometimes folks don't like the disk partitioning/file system setup - if
so you might like to try using the first Mandrake CD1

http://mirrors.usc.edu/pub/linux/distributions/mandrakelinux/official/iso/

They have a fantastic GUI based tool to accomplish this part of the
installation that covers nearly all aspects of this process. When done
with the partitioning you can return to Debian CD (but now the new
Debian installation process makes the whole issue of partitioning pretty
easy)

You should also take a look at http://www.linux-laptop.net/ to find out
any particular issues regarding systems prior to purchase.

The only problems I have encountered have been with the unsupported
wireless chipsets (check the laptop page noted above- i.e.things like
Centrino etc..).

The other thing has been failed Hitachi Travelstar notebook drives. This
of course is not related to the OS. I prefer using Seagate - they have
been extremely reliable (for our notebooks, workstations and servers)
but don't appear to be used in most notebooks or typical vendor systems.

I like Knoppix, but have found their disk installation facilities more
limited, but that may have changed in the last year.

Re: [Hardhats-members] IO file designation for VistA printing to CUPS server

2005-06-13 Thread Ismet Kursunoglu, MD

I just tried to pass the print job off using kprinter

http://printing.kde.org/overview/kprinter.php

using in my terminal settings

CLOSE EXECUTE: W *27,"E" U IO K IO(1,IO) C IO ZSYSTEM "kprinter
--nodialog --system cups -P HL1240 "_ION_" "_IO

Then sending a test job resulted in a much nicer looking output.

DEVICE: HL1240 HOME LAN

Do you want your output QUEUED? NO// (NO)
[ 2 pages * 1 copy ] left in /tmp/kde-ibk/kdeprint_sjs3cDRw
1 lines were wrapped
1 non-printable characters

Not sure what is causing the non-printable characters - I will look at
it in more detail but kprinter appears to take care of all the
housekeeping as far as creating (in my case emulating) PS output. And it
indeed looks much nicer!

K.S. Bhaskar wrote:
FWIW, I would advise against sending escape sequences or PCL directly to

send either straight ASCII text or, for nicer printouts, Postscript.
Since there is a postscript driver on Linux for (almost) every printer,
you are not tied in to a specific printer.

Mark,

I like your solution. It is certainly more succinct
than mine was.

From your code, it appears that IO is set to equal $I
before PRE-OPEN EXECUTE code it set. I didn't realize
that the variable ION was set to the device name.

We have a HP laserjet 5. I haven't tried sending it
escape sequences (as you seem to be doing.) When I
tried to choose subtype that was 12point, it didn't
print right. What are you doing with those escape
sequences?

Re: [Hardhats-members] IO file designation for VistA printing to CUPS server

2005-06-12 Thread Ismet Kursunoglu, MD

Mark, It prints!

Thanks very much. There is more progress as mentioned below.

*

I set the device name to the cups printer, in this case 'HL1240' with
proper close execute and it prints. There is a downwards "stair
stepping" in the printed output. Is this related to some terminal setting?

Kevin, I recall in an earlier post you mentioned this problem.
Is this something that can only be done with additional code or should I
just experiment with other terminal settings or designations? I have
not come across any document that directly addresses printing - i.e.

Ismet,

You should not need a POST-CLOSE EXECUTE, the CLOSE EXECUTE in the
TERMINAL TYPE will take care of it.
This field should be: CLOSE EXECUTE: W *27,"E" U IO K IO(1,IO) C IO
ZSYSTEM "lpr -r -l -P "_ION_" "_IO

NOTE the "_IO" on the end this varible is equal to the device name so
the Linux ZSYSTEM call for the lpr command will expand to something like:

lpr -r -l -P /home/ibk/t/HL1240.12345612345.TMP HL1240

You currently have the printer varialbe equal to "PRINTER NAME: HL1240",
the Device name needs to equal the Linux name for the printer. You could
just force the Linux name of the printer here just change the end to
"_ION_" HL1240

Also I have a space after the "-P" I believe some versions of lpr you do
not want a space.

Hope this helps.

Re: [Hardhats-members] IO file designation for VistA printing to CUPS server

2005-06-10 Thread Ismet Kursunoglu, MD
Thanks very much Kevin, you got me thinking more clearly about the whole
process. After making the changes according to your referenced note I
kept getting the unknown printer error as below:

OUTPUT FROM WHAT FILE: PATIENT ALLERGIES//
SORT BY: NUMBER//
START WITH NUMBER: FIRST//
FIRST PRINT FIELD: .01 PATIENT
THEN PRINT FIELD:
Heading (S/C): PATIENT ALLERGIES LIST Replace
DEVICE: test TEST-LINUX-PRINTER HOME LAN

Do you want your output QUEUED? NO// (NO)
lpr: HL1240: unknown printer

I installed the cups bsd compatibility (which has fixed many other
problems that I have had with cups printing configurations in the past)

# apt-get install cupsys-bsd

And it worked! Now I can go to town on the huge number of options that
VistA and Cups together allow for controlling the print jobs.
Outstanding thank you so much.

I guess there was some dependency in the lp system that was missing? Any
 one else experiencing this?

[Hardhats-members] IO file designation for VistA printing to CUPS server

2005-06-10 Thread Ismet Kursunoglu, MD

Kevin mentioned in one of his very helpful notes to this list,

the entries for the device file including the following:

PRE-OPEN EXECUTE: DO SETJOB^ZZZPRNTR(.IO) ;Note:

What should the output file be? Is that the spool directory for the
client, the URI of the printer or the type of file i.e. PDF/PS

I have setup CUPS on one of the LAN servers here (in this case running
under FreeBSD 4.8) and can print from all the Linux workstations as well
as from a Windows 2000 Pro box. I think I am almost there in printing

Sorry about being so terse: Look in the ALTERNATE EDITOR file to see
what code is invoked, and then look at the routine as a model.

Oh, no - not terse at all! It is just that I am not even in the third
violin section in this orchestra - at this point I am still sitting in
audience and applying to become an usher. :-)

2005-06-06 Thread Ismet Kursunoglu, MD

There is an ALTERNATE EDITOR file (I think I got the name right) that
provides a platform independent mechanism for invoking editors (much as

This is what I found. I am running GT.M V5.0-FT01 Linux x86 (under
debian-pure64 sid with emulation) and VistA FOIA20050227 from Bhaskar.

**
OUTPUT FROM WHAT FILE: ALTERNATE EDITOR//

   Choose from:

   1ACTIVATION CODE FROM DIWE

   3RETURN TO CALLING EDITOR

-A- SEARCH FOR ALTERNATE EDITOR FIELD:

Would it be possible to evoke ZEDIT and have it display for example with
Mailman or for that matter anywhere else in VistA? I would think not.

That was from the OpenForum server - I am not sure what systems they are

You should be able to use the same technique that is used to install

Maybe someone/they should rewrite this as "WorldVista, Fidelity and the

Re: [Hardhats-members] Languishing Wikki . . . What's up?

2005-05-12 Thread Ismet Kursunoglu, MD
We just this week completed the build out/physical plant for the Alaska
Clinic -i.e. now we can actually sit inside and see the magnificent view
of the Alaska range. And just yesterday did we got a static IP address
into the switch room. (BTW, we have 96 Ethernet ports to support the
2200 sq ft of clinic space- so the patch panel looks pretty impressive
all by itself!)

The main areas left to complete are the x-ray system (which is using a
high frequency generator, with three phase power) and then get all the
other equipment up and installed. The main element being the
exercise/pulmonary lab.

I have some information from Viasys/Sensormedics regarding their
products. Apparently they have around fifty interfaces with the VA's
Vista system with about thirty five of them are using the VA's Medicine
package, while the rest are using the new Clinical Procedures package. I
am getting much more information about their interfaces - but it seems
that these are really all based on receiving and sending HL7 messages. I
am not sure about the other equipment but I imagine it is the same i.e
for Echo lab etc..

I started the configuration of a Vista system using
VistAFOIA20050227/GT.M./Debian using some of mine and some of the docs
from the Wiki - these BTW were extremely helpful, along with all I
learned at the Boston meetings. WIth all this it appears that I could
become a Vista "addict"

I have been documenting all of the various areas as they come up -
including taking screen shots, and copying all of the text from the
various configurations. So far so good, but the notes will need lots of
editing. Also I have been creating some basic notes from the core
documents on things like Fileman commands and what not.

Hopefully this could in some measure be of help to others. Please let me
know.

Of course the question that comes up is if the OpenVista and OfficeVista
releases are going to have similar support for the various diagnostic
equipment. I would presume so.

I am doing my best to document the whole process (including all the
factors that go into opening a medical clinic and new business here in
AK) along with photos etc.. there is a ton of work to do, and I am
basically the only one up here at this point, so the going has been a
bit slow. But.. the good news is that Vista is there and the virtual
support from all of you has been nothing less than fantastic.

Kevin Toppenberg wrote:
I just looked at the change log at the WorldVistA

Shame on you all. Are we a community or what? Does
everyone have everything figured out and are happy
with the way things are? How are we ever going to be
able to welcome newcomers to our ranks--especially
when VistA Office comes out? This is not esoteric
documentation. It is simply recording real-life
solution to problems encountered, so others can avoid
our mistakes.

Contributions would be appreciated...

[Hardhats-members] CPRS 1.0.25.28 with Wine

2005-04-22 Thread Ismet Kursunoglu, MD
I did a fresh install/config using Bhaskar's VistAFOIA20050227 to a
server/demo laptop and tried to connect over a local switch here with
CPRS 1.0.25.28 (another laptop running Debian/Sarge)- it presents the
login and then complains about the now famous ActiveX control, and
finally hangs with this screenshot summing it all up -

http://www.alaskaclinic.com/cprswine.png
The server laptop is running Debian 2.6.8-10-amd64-k8 with
/etc/apt/sources.list being "deb
http://debian-amd64.alioth.debian.org/pure64/ sid main non-free contrib"
with the 32 compat layer installed.

Some of the output from the wine session follows. I know that Nancy and
Aric had worked on getting this to work and I was wondering if there was
any simple solution available? The wine version is 20050310 and as a
side note it is now running QuickBooks 6.0 perfectly on the same laptop.

I tried running the "fixed" version from Kevin's site
http://69.68.182.66/downloads/OpenVista/wine-CPRS
but after login there is a complaint screen about server/client
incompatibility. Screen shot at ->

http://www.alaskaclinic.com/serverclient.png
Is this generally the case with CPRS versions following the particular
Vista server release? Can the same juju be applied to CPRS 1.0.25.28? Or
does the Codeweavers product give me the "fix" for any version of CPRS?

I am still in the NYC area - I have met informally with a few medical
groups (small to large/enterprise) and talked in general about the
WorldVista project (in addition to many other topics)- so far there has
been a huge amount of interest. It is going to be very helpful to get my
new clinic up and running on this platform/solution - as seeing is
believing.

I was also thinking that it might be helpful, as others have mentioned,
to run a more full featured demo site in parallel as I bring up our
internal production system - i.e. one that would allow anyone to connect
and see what a working VistA system is like. I would be more than happy
to host such demo projects on the alaskaclinic.com site.

Thank you.

But we are not requesting a network connection - have no idea why that

Re: [Hardhats-members] Configuring FOIA Vista

2005-03-29 Thread Ismet Kursunoglu, MD
The complete statement was something along the lines of "This is a test
server for Vista" that was part of the GUI login screen for CPRS. I
don't recall which file it was that allows for the changes, it might
have been part of the CPRS documents from the HUI site.

Re: [Hardhats-members] DVD For backup... (more)

2005-03-27 Thread Ismet Kursunoglu, MD
So I am guessing that you have some automounter running or that the cd
(data I presume) was mounted as root either seperately or by k3b? I have
never tried to copy a cd or dvd using it.

Is the device seen in the usb bus? i.e. /proc/bus/usb/devices and is
there any special driver module that was being loaded that somehow is
now lacking? i.e. sometimes things like event device drivers or other
rare things need to be loaded prior to scanning the bus on boot or with
things like cdrecord

There are some graphical utilities to look at the usb devices in the
usbutils Debian package.

Is the DVD device on when you boot or do you also hot-plug the device? I
am sorry if I missed that part - but is the DVD burner powered from
additional AC or from the bus itself?

Is there something else - some other application / process that is
trying to use/access the device on bootup ? i.e. thinking that is is
going to play/view some multimedia file or the like?

And lastly is sg and scsi_mod modules loaded, or could it be that you
are only able to use it's cdrom reader capacity and not the dvd burning
ability due to some permissions/access problem.

What does the devices section from the k3b configuration show i.e. are
you running it with group permissions set for something like the dvd or
cdrom group?

You might also take a look at /var/log in syslog to see how and what is
being loaded when you plug the device in 'tail -f /var/log/syslog'

Once you get the device loaded you might try running k3b from the
command line and take a look at the output - you can see the options
with 'k3b --help-all'

I hope this gets you at least some more leads in getting it to work.

Kevin Toppenberg wrote:
Well, I have gotten k3b installed with everyone help.
The first few times I ran it, it was missing files
like growisofs, but the apt-get/yum system fixed that.

Then I ran k3b with a standard cd-rom in the dvd
drive. All was looking good. The DVD drive was
listed, and when I clicked on the drive, I could see
the cdrom contents. I wanted then to put in a blank
DVD. So I right-clicked, and selected "unmount", then
"eject".
That was when my server froze up. No mouse movement.
Nothing. I rebooted, and now the drive is missing.
Furthermore, during the bootup process, when kudzu is
looking for new hardware, it seems that the screen
gives a slight flicker that I don't remember before.

If I run lspci, I can see the USB 2.0 controller card
that I added to run the DVD writer. But the cdrecord
--scanbus doesn't show the drive like it used to. And
k3b doesn't show it.
I think this is a hardware problem. But I'm note sure
if it is the controller or the DVD drive. (The drive
was a SONY DRU ~720 and seemed of good quality).
Any thoughts?

Thanks
Kevin

Thanks to all (Kevin, Nancy, KS Bhaskar) for the tremendous help. I am

[Hardhats-members] Configuring FOIA Vista

2005-03-26 Thread Ismet Kursunoglu, MD
Hi,
I am trying to setup openvista from the following tarball from Sourceforge
OpenVistASemiVivAFOIAGold20050212.tgz
Using the directions from the Worldvista Wiki
http://openforum.worldvista.org/~forum/index.php?title=Installation_How_To_VistA_GT.M_Linux
I have the GTM> prompt but am not sure what documentation to follow for
configuring the rest of the system. I have tried this on both a AMD 64
(Debian sid) and Athlon XP 2000+ (Ubuntu Linux).

I successfully brought up a system using the HUI directions in the past
under a PIV i386 system with Debia (Sid) but now am trying to doing so
this AMD64 laptop - both from a whole new install/configure as well as
trying to migrate the older configured system (with all of the
networking, permissions/access and other details working).

I have all of the Debian IA-32 shared libraries for use on the 64-bit
system, and am assuming that with the GTM prompt I would be able to
load and run my previous environment. However I can't get to an access
prompt, yet I can get the GTM> prompt. (I altered the hostname and IP
address to match the setup on workstation)

I was wondering if there was a configuration document similar to
http://hardhats.org/projects/VistA_Install/Vista_Install.html to use for
GT.M?

We are wondering what access codes are available once one gets beyond
the basic setup and into the Vista install portions.

I plan on attending and am very much looking forward to the conference
in Boston. I am trying to get some of our engineers to attend as
well.(hopefully!)

Thanks very much.

Re: [Hardhats-members] Another option plus RE: Linux question: Setting up DVD for backup

2005-03-21 Thread Ismet Kursunoglu, MD
Thanks for your suggestions below. The one issue
about scp'ing the data, though, is that I would have
to have an "scp server" running on the windows box. I
don't know how to set that up. I have downloaded a
program for windows, pscp that can pull the data from
the linux box when run on the windows box. It would
be nice to be able to push it though.
Cygwin will give you the sshd part-
http://pigtail.net/LRP/printsrv/cygwin-sshd.html
and this might be a good solution to limit access to scp for your scripts -

Re: [Hardhats-members] Linux question: Setting up DVD for backup

2005-03-21 Thread Ismet Kursunoglu, MD

I need a reliable backup solution for my linux server.
Here are methods I have explored, unsuccessfully so
far. Let me first mention that our office also has a
new Windows Server running our billing package. My
initial thoughts were to copy my linux data to the
Windows server, and then let their backup process save
my data.
I just recalled another tool/method that you might want to look at -
namely rsync

Here is a nice summary of one method employing it with automated scripts:
http://www.tux.org/~tbr/rsync/rsynchowto.html
Rsync is a fantastic tool - especially when combined with ssh.
