Re: [Hardhats-members] specify LAB COLLECTION TIMES

2005-02-16 Thread Nick James
Hi
LAB COLLECTION TIMES can be specified by appropriately
editing the LABORATORY SITE file and the update the
CPRS OE/RR parameters through the following menu
options

LABORATORY DHCP MENU
   SUPERVISOR MENU
  LAB LIASION MENU
  OE/RR INTERFACE PARAMETERS
  UPDATE CPRS PARAMETERS

This will ask for Phlebotomy Collection Times, which
is actually the LAB COLLECTION TIMES.

Re: [Hardhats-members] Service/Speciality for Consults

2005-02-15 Thread Nick James
Nancy,
I thought i would write how i solved it in may mail,
but was wondering if anyone would find it useful!
Sorry.

This mailing list has been a source of great help and
support, i think it is something that i would find my
privelege to contribute to, if i can with the little i
know!!!

As far as the problem i had with not being able to
order the consult the set up Consult services option
in the Consult mangement option is used.
Here the service/specialty is to be specified (that
would show up in CPRS from the order consult)

As far as the setting up of the procedure is
concerned, the setup procedure option, again in the
Consult mangement option is used to enter the
procedure(s) to be ordered!

Let me know if i could help any further!

Thanks,
Nick

Greg,
To be more precise my question is (are):

1. when i send mail from for eg. yahoo to a user on
mailman, can the user receive the mail on their
mailman inbox? as far as i have understood, this is
possible?
So how do i configure mailman or OpenVistA on the
whole, so as to accomplish this.

2. how can i send mail from mailman to yahoo?

thanks

Re: [Hardhats-members] Transcription

2005-02-11 Thread Nick James
Kevin,
I managed to set zoc with KERMIT and tried sending a
word document. but i did not specify the proper format
of the document, i mean the ssn, and the format
specified.
will try that out as well!
Thanks a lot for your support
Nick
--- Kevin Toppenberg [EMAIL PROTECTED] wrote:

 Nick,

KERMIT will work from a linux terminal (I think),
 but
I have never done it this way. What happens with
 ZOC
is that you connect to your vista server like a
standard terminal emulator would. You then go to
 that
menu option TIU UPLOAD DOCUMENT (I think that is
 it).
then, when the vista server sends the 'ready' signal
(which in N3), then you you go to the menus in
 your
ZOC program and select KERMIT SEND. At this point
 the
ZOC program will bring up a file select dialog box.
Then you would select the file to upload. You'll
 then
see a progress dialog box that shows the file being
 transferred.

I should mention here that ZOC is capable of
 transfers
with KERMIT, XMODEM, and YMODEM. You have to go
 into
the session settings in ZOC and tell it that you
 want
your transfers to be in KERMIT protocol.

This KERMIT transfer stuff is a bit complicated. I
 am
figuring that out as I am trying to teach my
transcriptionists to do it (i.e. not very computer
savy people.) Its a bunch of steps, and sometimes
 the
KERMIT transfer fails and you have to do it again.
 I
am thinking about deriving a solution of somehow
sharing a drive/directory on my linux server with
 the
windows network. Then have the transcriptionist
 save
the file there. They could then do the upload via
 the
 HFS method.

Another solution that I have considered is to have
 the
transcriptionists create the note in their favorite
word processor. Then they could select it and
cut-n-paste it directly into CPRS. This would
 ensure
that they have the correct patient selected etc.

Let me know when/if you want more info.

 Kevin

--- Nick James [EMAIL PROTECTED] wrote:

  Kevin,

have downloaded ZOC. generally i work with putty
 so
this does look very different! wonder if putty is
  capable of KERMIT uploads!
I have understood what you have said. But that
 again
leads to another question! If i have the
 transcribed
document on my local machine how can VistA
 identify
that it is this specific document (out of the
  hundreds
of documents stored on my local machine) i need to
upload (using the KERMIT method)? Guess I'm
 missing
  out on something!
I've been able to enter the operation report, but
 i
had to do that as the CHIEF,MIS. Not as the
  transcriptionist!

  Nick.

--- Kevin Toppenberg [EMAIL PROTECTED] wrote:

   Nick,

   Sorry about that...

2.) Voice file/recording is assigned to a
transcriptionist.

This would take place independant of VistA.
 I.e.,
   in
my office, when I dictate a tape, it is given to
 a
transcriptionist. They know to then type it up.

  In
   a
hospital system, I think that the equipment
 would
   put
this into a que etc, ready for the next
transcriptionist to pull from. Again, this is
 all
   separate from VistA.

4. If word processor is used, then resulting
  file
   is
uploaded into VistA, and filer program puts
 the
   text

into a TIU DOCUMENT record.

This is where the rubber meets the road, so to
   speak.
Try selecting option: TIU UPLOAD MENU From that
   menu,
choose Upload Documents (option: TIU UPLOAD
DOCUMENTS). When you do, there will be about a
   10-20
second delay, and then some funny characters and
 a
   N3
are displayed. This is the signal that vista is
   ready
to accept a file for upload. It expects the
   transfer
to be a KERMIT transfer. So you have to have a
terminal emulator that is capable of kermit
  uploads.

I am using a demo version of ZOC

   which supports this.

I should mention, that Kermit upload is only one
  way
that the document can be transferred to Vista
 for
processing. One can also save the file in the
  host
file system... i.e. a standard file in a
 directory
somewhere. Then the upload process would
 consist
  of
providing the filename and path. You choose
 which
method you want in your TIU PARAMETERS.

After the file is uploaded successfully, the
   uploaded
file is processed. It scans the file, divides
 it
   into
the separate notes, and files each one. Before
 I
  go
into too much detail on this part, let me know
 if
   you
   are with me so far.

When i, as the author(^ZU) of the report, try
 to

the document, it states that the Elecrtonic

not yet enabled. But i have created the entry

   the
electronic signature in the new person file.

How does one enable the Electronic signature?
Wonder why the elecrtonic signature need not

Re: [Hardhats-members] Transcription

2005-02-08 Thread Nick James
Kevin,
I've got the odd steps (1 and 3),but steps 2 and 4?
how is that done?
how is a Voice file/recording is assigned to a
transcriptionist in VistA - the Menu's ... associated
with it?

As i was just looking up some of the documentation, i
came across some options that i do not have in my
installation of VistA. Like the Transcriptionist's
Menu, Surgery Transcription Menu... I found these
options from documentation that i had downloaded quite
some time back (Surgery User manual). This Obviously
was what probably existed before the SU*3.0*100 patch
was applied! This patch also, as one of its
requirements, had TIU*1.0*112 which i installed. Guess
the installation of this patch has to do with the
changes. Also the documentation of this Surgery
patch's user manual, does not talk of these options,
it has only a couple of pages on operation report
dictation which is obviously not very helpful.

Was just wondering if VistA has a dictation system?

I think this is an extension of a question on
 another
 post. Just a quick overview:

 1. Doctor dictates on hospital dictation system, or
 on
 a tape.
 2. Voice file/recording is assigned to a
 transcriptionist.
 3. Report is created by transcriptionist by typing
into CPRS, or by creating a report on a word
 processor.
 4. If word processor is used, then resulting file is
uploaded into VistA, and filer program puts the text
 into a TIU DOCUMENT record.

Re: [Hardhats-members] how to dictate an operation report?

2005-02-07 Thread Nick James
Kevin
I followed the steps you have given, that leads to
another question. When we click on the new note
button, it presumes that its a progress report and
hence you can only type in certain types of notes
(ADDICTION ASI-ADDICTION SEVERITY INDEX, ADVANCE
DIRECTIVE, ASSESSMENT WRIISC ASSESSMENT NOTE...).
But the operation report ??? Does this mean that only
these progress reports can be transcribed from CPRS?
Or are there some other settings to get the operation
report up?

Also the message YOU MAY NOT VIEW THIS UNSIGNED
NOTE. is what i get to see as of now from the tiu
menu (signed in as the transcriptionist and the
author). The problem is that the author of the report
is also not allowed to view it! I guess I'm missing
out on something!

Thanks

So basically it comesdown to how is the transcription

Re: [Hardhats-members] how to dictate an operation report?

2005-02-06 Thread Nick James
Kevin,
while i was trying to solve this problem,i figured
that i had not assigned the transcriptionist in the
transcriptionist user class (TIU). After doing so i
have assigned the 'tiu main menu transcription' to the
user. I can in the menu select the operation report,
but am neither allowed to view it Nor edit it. (either
as the transcriptionist or the author)the status does
not change from undictated. How do i link the report
to be typed to the transcriptionist?

Can transcriptionists type directly into CPRS?

Thanks
Nick.

 Nick,

I assume, since you are working with a hospital,
 that
you will be using transcriptionists. This is the
 same
issue I am working with right now.

Many transcriptionists prefer to type in word
processors, rather than into CPRS. They'll have
 more
tools at their disposal. So VistA is designed to
allow them to prepair a large document with multiple
patient reports in it. Each report has a separate
header. Then the file is uploaded to VistA. The
upload process then scans through the document and
identifies each report. Each report specifies the
type of note it is, i.e. SURGICAL NOTE, or PROGESS
NOTE etc. These note types are set up by you. Each
note type has a cooresponding filer program. This
 is
a program that prepairs a document entry in file
 #8925
(TIU DOCUMENT) for the filer to put the report into.

The TIU (Text integration utilities) module has some
sample filer programs that one can use to customize
for your location. I know that one of these samples
focuses on surgical reports. It somehow ties the
report to the surgical entity in VistA. I haven't
explored the surgical side of things since I am
 outpatient here.

This doesn't directly answer your question, but I
 hope
it gives you some background info that helps.

[Hardhats-members] Patient access to medical records.

2005-02-04 Thread Nick James

Was curious! As far as I know only a person from the
new patient file (provider...), has access to the
patient's medical record.
Can a patient access his own medical record? Does he
use CPRS? If so, how are the keys to be
assigned... security...?

[Hardhats-members] differential charging for diagnostic tests

can multople rates be maintained by VistA?

If the charges applicable for a test are to be based
on the patient type, and multiple rates apply for the
same test depending on the patient type.

If so how is it defined and which file is it stored in?

RE: [Hardhats-members] differential charging for diagnostic tests

2005-02-04 Thread Nick James
Hi Thurman,

Thanks, that is certainly helpful.

But the issues also relate to a categorisation of
direct paying patients which includes free (only some
select tests are free) subsidy, semi-subsidy, normal
private and luxury provate. The hospital where we once
worked for had six categories, each with differing
charges for the same test. I suppose one could treat
each category as a different employer using the same
logic as in industrial billing. Please comment.

--- Thurman Pedigo [EMAIL PROTECTED] wrote:

This is a little different philosophy that has
worked well for me over the
 past 30 years.

Billing for commercial insurance, Medicare, and
 Medicaid, have no need for
 multiple rate billing. Adjustments/write-offs,
 bring the payments into
compliance with the charge and provides a powerful
 tool to assess
 performance in the accounts receivables.

Industrial billing, where one contracts to with an
 employer to do specific
services at a set price uses modification of the
 last 2 characters in the
CPT code AND includes the company name in the
 description.

 Thanks,

I believe the majority of the billing is not done
 within VistA. The
  recent
billing patches, to my knowledge, only gather
 together information for a
third party billing service to use to process a
claim, and it may not even
include charges at all, as I believe that is
 probably handled by the
processing entity or information provided to that
 entity separately. Even
  if
there are charges withing VistA, I doubt there are
 any rates that would
  differ from patient to patient.

[Hardhats-members] Development stratergy for a new hospital

2005-02-02 Thread Nick James
Hi,
In the context of a new development in a 300-bed
hospital, what language and approach would you suggest
for those modules that are to complement VistA in a
local customisation. These relate to patient
registration, billing, pharmacy and other localized
user requirements that are not directly available in
Vista)?

Can it work independent of VistA, with only batch
updating of the VistA database? Then the system would
work with or without VistA.

The CPRS problem was due to the change in file access

Re: [Hardhats-members] Missing Surgery Tab... found (?)

2005-01-28 Thread Nick James
Hey,
I did manage to find that out as well. Actually I also
found out that in order to have the SR*3*100 patch up
i need the following patches installed
SR*3*96
SR*3*99
SR*3*105
SR*3*107
SR*3*112
SR*3*115
SR*3*126
TIU*1*112.
Out of these, i do not have (SR*3*126 and TIU*1*112)
installed on my server. and when i tried, i just
landed up sending a couple of mails to the group! I am
unable to install these patches on my system.
I tried doing the xu-8-275.rtns in the %RI at GTM. got
somewhere...???
but i still need help. guess its got something to do
with installing a KID file that I'm loosing out on!

THAT WAS WHAT HAPPENED WHEN I TRIED. After the
INSTALL/CHECK MESSAGE option on the PackMan menu

Select Installation Option: 1 Load a Distribution
Enter a Host File:
/home/vista/patches/SR-3_SEQ-120_PAT-126.kid

Released SR*3*126 SEQ #120

This Distribution contains Transport Globals for the
following Package(s):

I wonder if it has to do with Review your mapped set.
If the routines are mapped, they should be removed

Note: Until Surgery patch SR*3.0*100 is released,
sites will not see the Surgery tab.
When a user launches the application, CPRS checks
 to

Depending on the configuration of your site and

 reports

Nancy, you mentioned precident for adding tabs
 (i.e.
for the Peds/OB-GYN project). In this case, it is
 not
something that can be added at a server level. As
 I
look through the source code for CPRS, there are
 pages
that are hard-coded for this. So to add peds
growthcharts etc, you will need to do a little
 coding.
Not all that hard, just needs doing.

hi Kevin,

Thanks for your comments.

I am aware of the available 2 options which are the
same in our installation here. But when i was going

i came across this 'Surgery' tab.
Wonder if anyone has worked with it and has any idea

 there
are only 2 options: COR and RPT.

So I don't know how one would get a surgery tab set
up. Perhaps its an entire package not installed on
 my
 system.

Regarding the imaging stuff, the consensus I have
gotten from this list is to just leave it alone.
 It's
too much hassle (i.e. barriers of FDA regulation).

 Good luck

RE: [Hardhats-members] Cant add entries to Pharmacy Orderable Item

2005-01-24 Thread Nick James
Hi
I am able to enter the drugs into PHARMACY ORDERABLE
ITEM file through the option that you had suggested.
But still I am unable to order drugs from CPRS Chart.
The message shown is List index out of bounds (0).
Thanks
Nick

 Hello, Nick:

I was able to successfully get this to work a couple
 of summers ago,
when our company was setting up Vista. The drug
 file, which is a product
file, and the orderable file, which is a generic
 name/dosage form file
must be linked through the drug enter/edit function
 in the pharmacy
system. Once the product is linked with the
 orderable, the orderable
will appear in CPRS. (The assumption made here is
 that the version you
are using does have the orderable file in it)

You must assign yourself the proper keys to order
 drugs in CPRS, and you
must assign yourself all of the pharmacy system
 manager keys, so that
you can edit and match drugs for all types of
 orderables.

In drug enter/edit, the system takes you through the
 drug file, allowing
you to edit fields, match the drug to the NDF, mark
 the drug for the
proper pharmacy package (inpatient, outpatient, IV),
 and finally it
takes you to the orderable matching function, which
 creates the
orderable as viewed in CPRS. At this point you
 select the generic name
and dosage form that matches the product you are
 working on. Once this
is complete, that description will appear in CPRS as
 an orderable item.

One reason the system is set up this way is so that
 the physician does
not have to select a drug product when ordering in
 CPRS. The physician
orders by drug name, dose, and route. The
 pharmacist, as part of the
verification process, makes sure that the physician
 order matches a
product available for dispensing, and assigns a
 proper dosage
 administration schedule to the order.

Re: [Hardhats-members] clarification on Surgery tab in CPRS

2005-01-24 Thread Nick James
Kevin,
Thanks for your comments.
I am aware that the 2 options COR and RPT are the
same;), but while going thru some of the surgery
related manuals(i.e. surgery elecrtonic signature for
operative reports-implementation guide, version 3.0) i
came across this tab.

Has anyone come across thia and if so how does noe get
it.

 Nick,

As for as I can tell, the possible answers for types
of CPRS tabs desired is stored in field 101.13 of
 NEW
PERSON file (#200). This includes a pointer to the
file OR CPRS TAB (file# 101.13). In my system,
 there
are only 2 options: COR and RPT.

 Good luck

Also came across a GUI called 'VistARad'. As far
