I have a Milestone Microwave Tissue Processor and use Prowave from Milestone. It’s proprietary, but it has alcohol in it and does a great job on our tissue.I haven’t used mineral oil for years! [Histonet] Thermo's Excelsior Tissue Processor Program's
Hi All of you in Histoland, I am working with a facility that recently purchased a Thermo Excelsior Tissue Processor. They have had processing problems with several of their specimens using non-xylene clearing agent. These issues were with both bx's and larger tissues. The program and the non-xylene clearing agent was recommended by the technical staff at Thermo. The histology staff have switched back to using xylene verses the non-xylene clearing agent, and most of the issues have disappeared. Also, the histotech's were using reagent alcohol, histological grade. This grade of alcohol is denatured with methyl alcohol. The sales representative was in and informed us that this type of alcohol has damaging effects on the instrument. The representative will be coming in to flush out the system and reprogram the instrument next Monday. I looked over the current VIP program which is being used, and the program, timing and temperatures are a little different from most hospital and private laboratories I have worked with. We are going to use basically the same program for the Excelsior, that we use on the VIP. I would like to know what other Excelsior Tissue Processor users that use xylene have for their programs. It would be great to compare the reagents, programs, timing, and temperatures for Routine overnight runs and for Rapid Biopsy Runs. Thank you in advance for your assistance. [Histonet] Email Spoofed. addresses were highjacked
Hi All, The email below was sent to me in response to the Scam. I guess someone from histonet contacted him regarding the scam, which was spammed. It's interesting to find out how vulnerable we all are. Yahoo's customer support has had high volumes of similar problems. Akemi, Boy! Peter needs an Oxycontin! Maybe more than one. Don't worry about this. The correct term is being Email Spoofed. From my friends, it happens when some computer hijacker either creates content on a new or existing web site that you ended up clicking on. Peter suggests it is Where you visited, but it could be anywhere, even on a legitimate site. Some people I've talked to think Microsoft's web page is loaded with Spoof and Spyware! Sometimes, it is a computer worm or virus affecting your computer. Most of the time, it is associated with your email provider. [Most of my friends with Yahoo email have been Spoofed.] I would run a up-to-date anti-virus software, and (especially if that doesn't help) ask for help from Yahoo to quarantine or fix your address. You probably have it fixed already. If not, it will do it again until someone stops it. It IS only email. So what??? I did in NO WAY send this e-mail! Please do not in anyway answer or send money to the bogus e-mail scam which was sent in my name. THIS IS SCARY! I don't know what has happened, but I came home last night and my yahoo account sent me an e-mail which stated that my e-mail has been compromised for scam purposes, and if I didn't answer the questions below I would have my account frozen. I immediately replied and answered the questions, but then I thought, what if this is a scam to extract personal information, so I copied the the questionnaire and sent it to the yahoo alert center. I received a reply that they would check into it. This morning I couldn't get into my e-mail account so I had to jump a bunch of hoops and reset my password. I hope none of you have this happen to you. The Yahoo Account warning stated: Yahoo Mail has discovered series of illegal attempts on your Yahoo Account from bad Ip Location and will shut your account as it has been flagged as a spam account. Filling Correct Information Carefully and Sending to Yahoo Alert Center: I hope this was legitimate! The information they wanted was my user name, password and birthday. Hi All, I think all of you are missing the point of Patti's question. She stated that her lab was dinged for having plants in the lab by a CAP inspector. I had the same thing happen to me years ago. The inspector stated that plants attract insects that can contaminate a supposedly clean environment. Patti has an extremely well organized lab that only had a small phase (1) deficiency last year. I think the inspector couldn't find anything, so they had to come up with this ridiculous infraction. Hi Cynthia, Thank you, this is exactly what I was looking for. You are a rare jewel. Especially for the size of your facility. I am impressed! I wish I would have received more feed-back from small labs, but that's OK. I have been doing some pro-active sleuthing of local labs. After numerous conversations with my fellow histologists that do IHC, I am finding out what I have long suspected and have experienced while doing technical support and consulting. The routine histology laboratory is multitasking all of their regular histology duties with the addition of IHC testing. Pathologists do not want to lose the IHC revenue to a reference laboratory, so they bring on-board simple IHC panels, and send out their esoteric or prognostic markers to a well known reference laboratory. Most histologists have been asked to bring on board IHC, without the proper educational background or tools. The average histologist has not had the opportunity to take continuing education classes, or even go on the histonet. This may be due to lack of continuing education funds, or lack of interest; It is amazing that pathologists can go to their continuing education meetings, but don't support their histologists. Unfortunately, these histologists do not understand the importance of standardization or validation steps. When something goes bump in the night, they are unaware how to troubleshoot the problem. This is unfortunate for the histologist, the pathologist and most of all, the patient. One more reason for pathologists to support continuing education. Hope to see you all at NSH! Hi Rosa, Excuse me if I am wrong, but I think Bill is referring to using a microwave for special stains, and you are referring to a Microwave Tissue Processor. I have used an oven set at a higher temperature (85 -110 degrees C) for staining the silver portion of the GMS, and it generally only takes between 10-15 minutes. That is if you place the silver solution in the oven for approximately 10 minutes prior to use. I also use the oven set at 60-62 degrees C. for the bouins portion of the Trichrome stain. Although using a water bath at 60 degrees C. is the preferred method. You have more control using a oven than using a microwave. With a microwave, results vary with the # of slides placed in the coplin jar, as well as the placement in the microwave. There are variations in microwave intensities if you don't have a carousel. You also may have boil over of solutions. Food for thought. [Histonet] Reimbursement for IHC Tests from Medicare/Medicaid
Good Morning Histology Managers, The Chief Medical Director here at my facility proposed a question the other day regarding reimbursement for IHC testing from Medicare /Medicaid patients. I asked our billing manager, and she was going to investigate the matter, but I know my fellow histology managers out there in histoland are pretty up on these matters, so I am asking once again for your expert knowledge. The laws have been going back and forth regarding how much we can charge, and I have been out of the loop for awhile. Can we charge for more than (2) IHC tests for these patients? Let's say you want to do a panel that consists of (5) IHC tests, can we only get reimbursed for (2) of those tests? Any assistance would be greatly appreciated. Hi All, Anyone trying to reserve a room at the Doubletree should read this. I just tried to reserve a room for the upcoming Symposium. It is much gentler on the tissue, and is less likely to harm the cellular integrity. Dr. Todd Barry also prefers the use of EDTA formula's for BM Bx's. Below is the formula. or you can purchase from Biocare Medical. You should decal the specimen for 30 minutes to 1 hour and check for decalcification. DO NOT leave for any longer than 1 hour. 10% EDTA (Ethyl-media minetetra-acetic-acid Stock EDTA 10 ml DI water 90 ml pH 4.5-5.0 with Sodium Hydroxide If you prefer to purchase the IED Solution, Biocare Medical has it available. Ion-Exchange Decalcification Unit (IED Unit, Designed for Bone Marrow Biopsies) An advanced decalcification system that removes calcium from bone quickly while leaving superior cellular detail. [Histonet] Rotary Microtome's and the Meditome
Hi everyone in histoland, I am in the market for 3 microtome's. I was originally thinking of purchasing demo or refurbished Microm 355S units or comparable. I used this unit when I set-up a TMA lab and was very happy with it. I am in the process of demo'ing a Leica RM2255 unit. The Leica unit has all the bells and whistles the Microm microtomes has, but it is on a separate device vs Microm's features are created internally. I would like your thoughts on this. Also, I was given some information on a NEW Microtome called the Meditome Rotary Microtome. It is made in Germany by Medite and distributed by Micron, based in San Diego. It is my understanding that the engineers who made the Reichert Jung and the Microm microtomes developed this new microtome. It sells for under $10,000 and has a 2 year warranty vs the normal 1 year warranty. Although it doesn't have all the fancy features, it looks like a solid unit. Has anyone purchased one of these units, and if so, how do you like it? Hello, I responded to Gudrun personally, but I thought that the information I am supplying him below might be a good educational source for other histologists who are having similar scenario's for AFB contamination. I would also like to say that I have an extremely eager histology team that have expressed an interest in learning theory and practice. Most of my team are OJT. They have had several pathologists approach them with these concerns regarding AFB, and because of their knowledge, or lack of it, couldn't fix the problem. We are starting with the basics and I am giving in-services along the way. Hi Gudrun, They are shipping the recycled formalin directly to the outside hospital surgery to put their fresh tissues into. As far as the AFB, we have had intermitant (+) AFB on the edges of tissues, that has been extremely questionable. We currently are not cutting a negative (-) positive(+) control on the same water bath as the test sample. That will be remedied beginning Monday. The water here is also in question. I will be sending it out for microbacteria analysis. We are going through a thorough housekeeping and in-service process. We are starting with the tissue processors, which are currently only changed weekly (not rotated daily), embedding centers, which have to my knowledge, never been drained, and the hopper cleaned-out. The water baths are not scrubbed out with soap and HOT water daily and covered at night to prevent contamination. The water bath is not skimmed of excess tissue debris, each and every time another specimen is placed on it. The Carbol Fuchsin is not being filtered prior to use. The working solution is currently being poured back into the stock bottle. Thanks Freida, What better source to validate the steps I am taking to eliminate the problem of AFB Contaminate. I would also like to mention in my haste to compose my e-mail, I may have stated something incorrectly. I did not mean that a positive (+) control be cut at the same time as the test sample and negative (-) control. I usually place the test tissue on the same pre-cut positive (+) control slide. We centrifuged tap water in 50-ml tubes, poured off the supernatent, and refilled and recentrifuged until we could see some sediment in the bottom. We took that to Microbioloby and had it cultured to definited prove the presence of AFB in the tap water. I would recommend this approach to anyone suspecting tissue contamination. Don't forget the effects on decalcified Bone Marrow Cores. Even if you use EDTA decalcification solution, there will be diminished FE staining on Bone Marrow Cores or any other tissue that is decalcified and then stained for FE. Hi All, I guess I was stupid to link on this file. I just hope aa aa didn't do this as an act of malicious behavior, and now we have viruses in our computers! I usually delete any unknown sender, and fortunately my MAC filters out most spams. Now I will have to do a diagnostic check. I recommend any of you who foolishly opened the link to do the same. Bernie, I used to use the method below for FS on muscles at Emanual Hospital in Portland OR. Since we had such wonderful success, we incorporated this method for all FS. Years later, I developed the MATSSE (pH3.4) 1-Step Trichrome Stain Kit (Modified Trichrome Method for Muscle Biopsies Cut on Frozen Sections) for Biocare Medical. This kit has long since discontinued by the way...This came from Biocare's Data Sheet. NOTE: Muscle tissue should be suitably obtained and frozen within 30 minutes after excision. The usual method of freezing is to first mount a transverse section of the muscle on a chuck using 10% tragacanth gum or equivalent commercial frozen section mounting media as the adhesive. The chuck with the mounted specimen is then immersed in prechilled isopentane until frozen. Isopentane is precooled when a container of the substance is placed into liquid nitrogen. At a temperature of -160° C, the isopentane has a slightly syrupy consistency. Care should be taken to remove the muscle sample when freezing is complete, usually after 25 to 30 seconds. Too short a freezing time produces artifacts; prolonged freezing can produce cracking of the block. NOTE: If the sample cannot be sectioned immediately after freezing, it may be wrapped in foil and placed in a plastic-lidded container along with a small amount of ice for moisture. Specimens may be stored at -70° C until sectioned. Sorry, I too noticed that the text looked weird then I did a cut and paste. I keep all the Data Sheets I created in my files, and this was just a portion of the Notes from the original Data sheet. I thought it might be helpful because you stated you had difficulty with cracking. You did not mention using isopentane. Some people immerse the tissue straight into liquid nitrogen, which could cause freezing artifacts. [Histonet] Sara Sorry to see you go
Hi Sara, I for one will be sad to see your absence. Perhaps in the future, you will reconsider and join our group again. It is extremely unfortunate to see these grown men acting like FOOLS! I have been pretty much a voyeur as of late, unless I have something credible to say or ask, but I feel the urge to state my feelings as a fellow histologist. And by the way, there are a number of extremely prominent histologists and pathologists that have stated they are voyeurs too! They may not go on line, but they see your postings! They have all taken note of this stupid on-line communication. I don't think these individuals realize that the pathology community observes these communications, and this may come back to bite them in the butt later on down the line! It lowers the image of these individuals as professionals, and their names and verbiage are forever archived in our memories. Once you ring the bell, you can't un-ring it! The Histonet Forum was initially created and funded by histology vendors and concerned histologists to help us as a group. I maybe wrong, but I thought it's purpose was to expand our knowledge in this valuable profession. I have been utilizing this wonderful tool, since almost the beginning. It was an eye opening experience when I started seeing pathologists joining in. It made me realize that they too saw the merit in this wonderful tool for sharing our knowledge and experiences. I have been involved in histology since taking classes in histomicrotechnic at Tokyo University in 1968. The older I get, the more I realize how little I know. Especially with the use of molecular biology, and more and more esoteric testing. We are experiencing a metamorphosis in our field, and forums such as this, are extremely valuable! Those of us who use this tool, do so, because we care about giving the best possible patient care. At least I use to think so This is a wonderful tool for histologists, researchers, and concerned pathologists, to come together for the sole purpose to learn and grow (then there's the Friday good hearted release of steam) The Histonet is especially important for those individuals who are geographically isolated, and do not have the means to attend continuing education classes. It is my belief that the Histonet was never intended to be used as a tool as these individuals have so irresponsibly done. It is childish and unprofessional. This sort of use just shows how unthinking and selfish these individuals are. I am ashamed that the pathology community sees this sort of communication between histologists. It demeans us as a whole. I don't think these men realize what fallout may occur. We may have this taken away from us because they have not used their good judgement, common sense, and restraint! I realize that in the past, I too may have spoken a little out of turn, but this thread has become an embarrassment. I have recently accepted a employment opportunity to go back to managing a histology lab, and will ask my fellow histologists for their assistance. I truly hope there will be a Histonet available when I need you! Let's hope we can learn from this experience and move on. Hi All, I have been receiving numerous inquires as to why I have left PhenoPath. I appreciate your inquires and congratulations. I am going to give a blanket reply, since I have time constraints. PhenoPath is a great company and I have the highest regard for Dr's. Allen Gown, Steve Kussick, and the rest of the staff. Sometimes there is a bigger plan in the cards for us, and my talents are more suited for the laboratory and direct patient care. I was offered a wonderful job in Los Gatos, CA, which is 30 minutes from Santa Cruz, and an hour from Carmel and the wine country. I love the Bay Area. I lived there for 8 years while working for Biocare as the Director of Histology, and I have a lot of friends there, plus weather is wonderful, and it's like going home! Hi All, I totally agree with Robert on his information regarding periodic acid and chromic acid. I have done extensive research on this subject when I developed my special stain kits for Biocare Medical. I did incorporate 10% chromic acid for the GMS stain, and 1% periodic acid for the PAS stain. As far as this forum for political views, in frustration, I deleted everything with these headings. There could of been some very interesting feed-back, but you have used this source totally inappropriately! I believe in this countries beliefs for freedom of speech, but use it wisely!! So your first step is to see if you're actually using chromic acid. PLEASE get the politics off this list - the repetitious garbage is making the list hard to read, particularly in digest form.
