Hi Guys - working with a little lab and we're starting from four walls and a floor. It is a human tissue lab but we won't have access to human tissues until everything is set up and validated. [Histonet] Is there a way to orient FFPE sections with an indicator that remains visible post staining?
Hi everyone- We have a researcher who needs to put multiple samples in one block for quality and cost control. There are four different samples per tissue and they've been processed differently but all are FFPE. Is there a way to put an orientation mark in the block that transfers to the slide - like a section-able plastic bead or wax-based dye so we can embed these membranes in a specific order and she can keep track of the order on her future slide preps? I recall there was a cassette with little plastic 'fingers' to orient shave biopsies to embed later where the plastic cut and remained on the slide without interfering with any staining process or coverslipping- something like that?? Help? we have a student with one of the online schools and we're doing specials like it is 1972. She has to do a Al Blue 2.5 w Hyaluronidase digestion. We haven't done this since the development of clinical IHC two dozen years ago - Is there any other way to acheive this outcome on a colon slide or do I have to order the reagent? Hi Joe-- are these research protocols or fda approved tests? If you're not charging the patient's insurance you're not restricted to CPT codes for billing. You can CLIENT bill them a reasonable 'per curl' charge or you could bill the handling charge and release the block so they can cut their own. Consider how clean you have to keep everything for this-- a couple of transferred cells and ... We require patient signature or signed clinician orders from the requesting entity and release the block with really really detailed tracking including scans of the release document saved with the primary case. Hi Terri - That list is specific to New York State, no other state is so restrictive, and it doesn't cite 'received date' as a requirement. If you've got all that other stuff-- which is generally included on reagent manufacture labels with minor exception-- I can't think of what value a received date would add. I'd not go to all that effort without a clarification from the state. Most inspection agencies aren't trying to create work for no gain. The takeaway? With all the licensure restrictions, state-specific regulations and remarkable amounts of snow- y'all have it ROUGH! Hi guys- I'm probably coming in a little late to this conversation. Received dates are no longer required by any of the governing bodies. Haven't been for a while. Opened dates aren't required either, as long as the reagent's performance isn't altered by the mechanical act of opening the container (this does apply to some clinical reagent's and almost no histology supplies when stored correctly) The only required date is expiry. If on the rare instance the manufacturer doesn't supply one, your institution would need to decide what works (1 year up to as many as 5 years) and include this statement in a policy. Then and only then would you need to mark the incoming supply with the received and internally determined expiration date. To complete the variables that may come up- we can no longer extend and expiration by freezing. This used to be a common practice for concentrated antibodies to extend expiration, but no more. Take a minute and look it up for CAP, JCACO and all the others-- it'll save you SO much time -- don't forget to update your policies so you're not breaking your own internal guidelines. The only reason I can think of to date everything is if folks don't reliably rotate stock-- and even if they are-- if your supplier isn't doing it well, your applied dates might not line up with first expired-- Thoughts? [Histonet] aqueous mounting media (gelatin)
Hi Jorge- There are numerous commercially prepared aqueous-based mounting medium. Most clinical versions are used for immunoflorescent staining. They're ExPEnSiVe!! When we use them we dip the slide or add a drop or two of the most recent rinse solution. For our IF mounts (also temporary) we dip in Ventana's reagent buffer so there is quite a bit of liquid on the slide, then drip a few drops of mounting media right on top of the tissue (centered) and drop the coverslip on flat (not from the edge). Press down in the center right over the tissue with a gloved finger. This pressure from a finger drives the more mobile components (bubbles) to the edges away from the tissue we want to see. It's messy-- you get all sorts of goop out the side of the slides, but these are easily cleaned with a gauze pad soaked in buffer and then left laying flat to dry for a little bit. Additionally-- thin your gelatin a little so it doesn't get fully firm when cool - with a temp mount the additional liquid won't make any difference except to allow your bubbles to move more freely. If you need them to last longer without evaporating, you can seal the edges of the slides with fingernail polish or regular mounting media (resinous) left to fully dry. Hope this helps! Kim- Your histonet question may not be as complicated as it might seem. Sometimes it's easier to look at these things backwards. What is the desired outcome? If there is an order -- say for a GMS--and it wasn't ordered by one of your pathologists, where did it come from? Can you track back and figure out what doctor ordered it and verify it's a valid request so the testing AND billing is appropriate (not fraudulent). When the surgeon or clinician collects the sample at the surgery or in their office, sometimes they want something specific -- say 'evaluate for fungus'. They may include this in the surgical notes, the office chart -- other places. His support staff will copy this onto the requisition or somewhere you get the request other than the requisition. If you keep copies of the req and other incoming documentation-- you've satisfied the requirement--you can track the source of the order. If you don't, include it in the gross description or notes that are transcribed onto the report so that you have a durable record that you can find (may take a while if it's the archived chart, but you can find it). This goes back to the requirement that orders can't just come from anyone or for any wild-hair reason-- and you have to be able to substantiate or prove the valid source of an ordered (and billed) test. [Histonet] marking tiny specimens
I am processing some extremely small specimens - pin tip size. These are eggs with a protein covering. I have tried using eosin to color the tissues before processing but the color came out before paraffin. The coating on the eggs will not absorb the dye. Does anyone have a suggestion for dyeing or marking these tissues so I can see them better to embed. Recently I needed to mark some previously fixed and decaled bones. I had neither tattoo ink or India ink. As an alternative, I rinsed the tissue, blotted it dry and marked the surface with a fine point cassette/slide marker. I use both StatLab and Leica. It worked like a charm. Because the ink is permanent there was no problem losing it during processing and the fine points left just small dots. The only thing is that the tissues should be blotted well as moisture seems to cause a little bleeding. All of our other chemicals had expiration dates, but not these. CAP cited us, we sent this article in: Stain and dye stability over a 30-year period: a comparison of certified dye powders by the Biological Stain Commission DP Penney, M Frank, C Fagan, C Willis Biotechnic Histochemistry 2009, 84(1): 1115. Hello all - I have been volunteering in a lab with very old equipment. Although I was able to find 2 processors, they have both died. We do not have the money to buy even a used processor, so I am looking for refurbished parts. [Histonet] Light green for PAS
Michelle - I agree that the light green is dead and you should make some new or buy a commercially made solution. However, if you want to try a different counterstain try a light hematoylin (usually used for glycogen) or metanil yellow. Either will give good contrast to the pink of the PAS. Hi Ann- May we assume you've confirmed this is happening at embedding and have ruled out any floaters happening during cutting? When embedding, keep Kimwipes or other tissues around, keep the wells closed and only pull out enough cassettes that you can keeep clean and clear of your working area. Wipe forceps and surfaces between blocks containing fragmented or friable tissue, don't put forceps back in the wells without wiping. You can stack a few guaze pads on top of the spout to wipe as you replace the forceps and change the pads frequently. Always, always only open one cassettte at a time and never leave the station with an open cassette on the station. Finish before standing or recap and replace. If you are working with currettes, cell blocks, or other cellular, friable tissue, open and unwrap on a clean surface (hot or cold - embedding station surface or wipe or l'absorb) and don't reuse the surface before wiping or replacing. If you're using knives or scalpels to scrape, make sure the handles and connection points aren't harboring residual tissue. Buy those little seamless paring knive from the dollar store--they fit in the wells and wipe easily. Use swabs between embedding sessions or between people trading places to clean the wells and then clean them again at the end of the embedding session. If you keep molten paraffin in the hold bins, filter or replace frequently and do not reuse. If you keep the hold bins dry, clean routinely (daily) Clean your molten wax chamber periodically to remove contaminants and keep the filter from clogging over time. Most embedding station mfc don't condone running xylene through the tubes pumps--clean hot wax will do the job. We always make it the responsibility of each person to clean both at the end of embedding AND to clean again before starting to fully assure the wells and surfaces were clear and eliminate a possible cross if one person in the chain forgets...double system processes like you double check specimen IDs. You'll go through a whole bunch of kimwipes--but they are much cheaper than gauze and SOOO much better than a cross contamination situation. IF it still happens, it's time to track who embeds each block to see if there's a pattern by person. The point is not to write people up but to support developing clean habits and to adjust their habits to do it to their best ability. Wipe wipe wipe wipe wipe!! Hope this helps!
