Hi and help?  A tech on another forum is dealing with a catastrophe. 

Does anyone have the glycerin de-mummification procedure?  Haven’t used it in 
decades but with what those samples went through it may be their last chance as 
any sort of readable slides.

Does anyone have a reference for how long the Dako Artisan refrigerated kits 
can be at room temp?

Also - having inconsistent increase in background in GMS.  Not sure if it’s the 
slides or the kit.  We’ve heard about the methenamine shortage -  do y’all know 
anything more about this kinda problem??

[Histonet] startup lab validation materials - suggestions welcome!

Hi Guys - working with a little lab and we're starting from four walls and a 
floor.  It is a human tissue lab but we won't have access to human tissues 
until everything is set up and validated.
I've read everything I can find- any thoughts on how to obtain tissues to test 
from processing through simple stains so we can get our program going?  It's 
all FFPE stuff.

GS 9 and no, you do not need a FL license for the VA.
You do however, have to have an ASCP (American Society for Clinical Pathology) 
HTL (Histotechnologist) license.

[Histonet] Is there a way to orient FFPE sections with an indicator that remains visible post staining?

Hi everyone-
We have a researcher who needs to put multiple samples in one block for quality 
and cost control.  There are four different samples per tissue and they've been 
processed differently but all are FFPE.  
Is there a way to put an orientation mark in the block that transfers to the 
slide - like a section-able plastic bead or wax-based dye so we can embed these 
membranes in a specific order and she can keep track of the order on her future 
slide preps?
I recall there was a cassette with little plastic 'fingers' to orient shave 
biopsies to embed later where the plastic cut and remained on the slide without 
interfering with any staining process or coverslipping- something like that??

Does anyone have a cool visual chart or metric that drills down including CPT 
reimbisrsement through cost per block/slide with tech and PA time in the 
production accounting?

If my memory serves, Rene Busa had a plethora of great info in this realm - are 
you still hanging out, sir??  Would love to hear from you

[Histonet] Alcian Blue digested w Hyaluronidase - any alternatives?

Help?   we have a student with one of the online schools and we're doing 
specials like it is 1972.  She has to do a Al Blue 2.5 w Hyaluronidase 
digestion.   We haven't done this since the development of clinical IHC two 
dozen years ago -
Is there any other way to acheive this outcome on a colon slide or do I have to 
order the reagent?
Even your best guess is welcome as we didn't get ahead enough to order the 
reagent and she'll end up submitting a null response (and it will be largely my

Good morning 'netters!
My sister lab is seeking a specialty histotech - more in line with research 
type work.   Day shift, flexible hours and VERY well compensated.   Will need 
to start quickly ( in 2-4 weeks).
Frozen section work with a dissection microscope, IHC and some routine and 
specials staining and a lot of detailed microscope work. Work needs to be 
PERFECT - not fast.  
If you're a detail-oriented person who gets jazzed by creating gorgeous slides 
- this is for you.
Open to MTs who can cut, as well as HT/HTL applicants.   Training for specific 
techniques and reading protocols provided.   Will eventually grow to a team of 
4-5 techs in the unit.  The first tech in will help build out, set up and 
validate -  I'll be your partner for the build and ramp up to production.  It 
will be fun and rewarding - love that awesome feeling of "I DID THIS!!"

model but it is SO BiG.
We need 3-5 colors of cassettes, and it has to network so we can have multiple 
users.  We print two lines (number and name) with hopes to go to full bar 
coding in the near future.

[Histonet] Histology Lead position - help!

Houston, we have a problem!
We need a Histologist Lead tech.  My acting lead is leaving us to take care of 
aging family in another state (love that she can make this choice).  This 
leaves us with a hole in our staff that really needs filled up fast!
Have to be gross qualified-- we can teach and it's mostly derm and GI -- you do 
need to have the base education. 
And registered--either HT or HTL.  
It's not a complicated lab-- and we like each other and have fun most days.   
Our new facilities will be ready in August.

Hi Joe-- are these research protocols  or fda approved tests?  If you're not 
charging the patient's insurance you're not restricted to CPT codes for 
billing.   You can CLIENT bill them a reasonable 'per curl' charge or you could 
bill the handling charge and release the block so they can cut their own. 

Consider how clean you have to keep everything for this-- a couple of 
transferred cells and ...

We require patient signature or signed clinician orders from the requesting 
entity and release the block with really really detailed tracking including 
scans of the release document saved with the primary case.

[Histonet] pH of stainer waste water flow

Hi guys- anyone know the overall pH of waste water coming off an automated 
stainer?  Yeah, I know different models and solutions-- but a general response 
(acid/base/neutral) for a whole process would be helpful

The city tested our run off and after 4+ years with the exact same process our 
waste is coming out wy too acidic.

[Histonet] Giemsa control

The best control for a non-blood Giemsa stain is a mast cell tumor from a
canine.  You can probably get a block from the Univ of Penn Vet School.  It
is really close to you.  The mast cells will stain purple or magenta.

Hi Terri -

That list is specific to New York State,  no other state is so restrictive, and 
it doesn't cite 'received date' as a requirement.  If you've got all that other 
stuff-- which is generally included on reagent manufacture labels with minor 
exception-- I can't think of what value a received date would add. 

I'd not go to all that effort without a clarification from the state. Most 
inspection agencies aren't trying to create work for no gain.

The takeaway?  With all the licensure restrictions, state-specific regulations 
and remarkable amounts of snow- y'all have it ROUGH!

[Histonet] received dates no longer required

Hi guys-

I'm probably coming in a little late to this conversation.  Received dates are 
no longer required by any of the governing bodies. Haven't been for a while.  
Opened dates aren't required either, as long as the reagent's performance isn't 
altered by the mechanical act of opening the container (this does apply to some 
clinical reagent's and almost no histology supplies when stored correctly)

The only required date is expiry.  If on the rare instance the manufacturer 
doesn't supply one, your institution would need to decide what works (1 year up 
to as many as 5 years) and include this statement in a policy. Then and only 
then would you need to mark the incoming supply with the received and 
internally determined expiration date. 

To complete the variables that may come up- we can no longer extend and 
expiration by freezing. This used to be a common practice for concentrated 
antibodies to extend expiration, but no more. 

Take a minute and look it up for CAP, JCACO and all the others-- it'll save you 
SO much time -- don't forget to update your policies so you're not breaking 
your own internal guidelines. 

The only reason I can think of to date everything is if folks don't reliably 
rotate stock-- and even if they are-- if your supplier isn't doing it well, 
your applied dates might not line up with first expired--

Not bleach!  It'll change anything with melanin and other cellular component. 

Try formalin vapor (in a chamber in small batches).  Or if you must rinse them, 
10% neutral buffered formalin or whatever your original primary fixative was 
prior to processing.

[Histonet] aqueous mounting media (gelatin)

Hi Jorge-
There are numerous commercially prepared aqueous-based mounting medium.  Most 
clinical versions are used for immunoflorescent staining.  They're ExPEnSiVe!!  
 When we use them we dip the slide or add a drop or two of the most recent 
rinse solution.  For our IF mounts (also temporary) we dip in Ventana's reagent 
buffer so there is quite a bit of liquid on the slide, then drip a few drops of 
mounting media right on top of the tissue (centered) and drop the coverslip on 
flat (not from the edge). 
Press down in the center right over the tissue with a gloved finger. This 
pressure from a finger drives the more mobile components (bubbles) to the edges 
away from the tissue we want to see.
It's messy-- you get all sorts of goop out the side of the slides, but these 
are easily cleaned with a gauze pad soaked in buffer and then left laying flat 
to dry for a little bit.
Additionally-- thin your gelatin a little so it doesn't get fully firm when 
cool - with a temp mount the additional liquid won't make any difference except 
to allow your bubbles to move more freely.  If you need them to last longer 
without evaporating, you can seal the edges of the slides with fingernail 
polish or regular mounting media (resinous) left to fully dry.

[Histonet] Plasma/thrombin clot for cell block

Your histonet question may not be as complicated as it might seem. Sometimes 
it's easier to look at these things backwards.  What is the desired outcome?  
If there is an order -- say for a GMS--and it wasn't ordered by one of your 
pathologists, where did it come from? Can you track back and figure out what 
doctor ordered it and verify it's a valid request so the testing AND billing is 
appropriate (not fraudulent). 


When the surgeon or clinician collects the sample at the surgery or in their 
office, sometimes they want something specific -- say 'evaluate for fungus'.  
They may include this in the surgical notes, the office chart -- other places.  
His support staff will copy this onto the requisition or somewhere you get the 
request other than the requisition.  If you keep copies of the req and other 
incoming documentation-- you've satisfied the requirement--you can track the 
source of the order.  If you don't, include it in the gross description or 
notes that are transcribed onto the report so that you have a durable record 
that you can find (may take a while if it's the archived chart, but you can 
find it).

This goes back to the requirement that orders can't just come from anyone or 
for any wild-hair reason-- and you have to be able to substantiate or prove the 
valid source of an ordered (and billed) test.

special storage. 
What supplier do you use and does your formalin have little to no methanol -- 
with a flash point higher then 200 degrees?

[Histonet] marking tiny specimens

I am processing some extremely small specimens - pin tip size.  These are 
eggs with a protein covering.  I have tried using eosin to color the tissues 
before processing but the color came out before paraffin.  The coating on 
the eggs will not absorb the dye.  Does anyone have a suggestion for dyeing 
or marking these tissues so I can see them better to embed.  Thanks in

[Histonet] India ink

Recently I needed to mark some previously fixed and decaled bones.  I had 
neither tattoo ink or India ink.  As an alternative, I rinsed the tissue, 
blotted it dry and marked the surface with a fine point cassette/slide 
marker.  I use both StatLab and Leica.  It worked like a charm.  Because the 
ink  is permanent there was no problem losing it during processing and the

We are about to run out of AFB control slides and haven't had a good loaded 
case in a while.  Is there an easy way to come up with an AFB positive block or 
could someone lend me one to be replaced at a later date? (Go through too many 
to be cost effective to buy)

[Histonet] looking for techs in Louisiana

Hello everyone--happy July 4th!
 
Two labs in SW Louisiana are looking for histotechs.  One needs PRN so anyone 
within driving distance who wants a pick-up job--give me a holler?  I'm doing 
this as a favor so it's not a recruiting thing--helping a friend.
 
Also--My lab needs a night shift tech--possibly a lead position with IHC skills 
but we'll consider a good core routine histotech as well.  We have fun, the lab 
is clean, with good equipment and reasonable workloads. We even go out as a 
crew once in a while--a rare culture these days. The most recent hire couldn't 
manage the shift (her first time on nights).  Also not a recruiting thing--it's 
my lab!!

[Histonet] Talk to me about shelf life and outdates

We're prepping for an inspection and I have a small amount of older powdered 
dyes and substances we can't get any more.    They're dated 'Good until used'.
 
Would someone elaborate on how we came to have outdates on things that never 
had them before, and how those outdates are determined?
 
Is there a procedure or policy you might share so we can keep our old but good 
items in service until we run out rather than in 12 months (EEEK!!!)

Is there a reference lab or facility that takes reference cases for eye 
pathology, specifically corneas?  Do they bill Medicare?

Nice.  So they clearly state UNFIXED but leave out FIXED tissue--which allows 
the assumption that fixed tissue is not on the list thus not biohazardous.

Hazard Recognition
The CDC estimates that 5.6 million workers in the health care industry and 
related occupations are at risk of occupational exposure to bloodborne 
pathogens, including human immunodeficiency virus (HIV), hepatitis B virus 
(HBV), hepatitis C virus (HCV), and others. All occupational exposure to blood 
or other potentially infectious materials (OPIM) place workers at risk for 
infection with bloodborne pathogens. OSHA defines blood to mean human blood, 
human blood components, and products made from human blood. Other potentially 
infectious materials (OPIM) means: (1) The following human body fluids: semen, 
vaginal secretions, cerebrospinal fluid, synovial fluid, pleural fluid, 
pericardial fluid, peritoneal fluid, amniotic fluid, saliva in dental 
procedures, any body fluid that is visibly contaminated with blood, and all 
body fluids in situations where it is difficult or impossible to differentiate 
between body fluids; (2) Any unfixed tissue or organ (other
 than intact skin) from a human (living or dead); and (3) HIV-containing cell 
or tissue cultures, organ cultures, and HIV- or HBV-containing culture medium 
or other solutions; and blood, organs, or other tissues from experimental 
animals infected with HIV or HBV. The following references aid in recognizing 
workplace hazards associated with bloodborne pathogens.

I was supervisor of a lab where we had old staining powders that were close
to 30 years old.  All of our other chemicals had expiration dates, but not
these.  CAP cited us, we sent this article in:

Stain and dye stability over a 30-year period: a comparison of certified
dye powders by the Biological Stain Commission
DP Penney, M Frank, C Fagan, C Willis Biotechnic  Histochemistry 2009,
84(1): 1115.

And they expunged the citation.  This is not an official advice, your
experience with this may vary.

Job Opening:  Histology Tech II
 
Reports to: Histology Manager
 
Core Description: Full-time, non-exempt position.  Performs all duties of a 
certified histology technician in preparing tissue specimens for pathologist 
examination: grossing, embedding, cutting and staining formalin-fixed tissue in 
an accurate and timely manner, in accordance with standard histology practices.
 
 
Must be a team-player, maintain a cooperative attitude and support on-going 
changes in a professional manner.
 
Schedule: To be determined.  Work schedule may adjust to meet the needs of the 
department.
 
Requirements:  Registered HT or HTL(ASCP). Associates Degree or equivalent with 
emphasis in science.  Preference given to individuals with 2+ years of IHC 
experience in a high-volume lab. Bachelor-degreed applicants encouraged to 
apply.
 
Duties include: Embedding, cutting, special stains, immunohistochemistry, 
troubleshooting, maintenance and support duties. Possible grossing and cytology

Hi All-
 
I'm working as a manager in a lab and we need some refurbished equipment.  
Vendors and refurb contractors please feel free to email me directly-  
 
1. Cryostat; basic but well-sealed.  For surgical use so we don't need all the 
bells and whistles of a Mohs unit but it will be remote so low maintenance is a 
plus. THIS IS URGENT--will purchase within two weeks.
 
2.  Paraffin pot - large.  Up to 5 gallons with a small footprint
 
3. Centrifuge: must handle 50ml conical tubes.  For cytology non-gyn fluid prep.

[Histonet] short temp but home for Christmas

Looking for a happy person and a good cutter, preferrably the same person!  
 
A short day shift temp, M-F from ASAP through the Friday before Christmas.  
Fill up the bank account and be home for the holidays.  Experience with small 
biopsies required.  Full travel including per diems, hotel, flights and car 
rental (or reimbursement for using your own vehicle).

[Histonet] Need parts for old equipment

Hello all - 


I have been volunteering in a lab with very old equipment.  Although I was 
able to find 2 processors, they have both died.  We do not have the money 
to buy even a used processor, so I am looking for refurbished parts.  I am 
hoping there are some vendors out there who have parts that we need or if 
there is a lab with a processor that works that they don't want we might be 
able to pay shipping.  

  

I have a VIP 3000 and need a motor, gear head, bearing washer and roller 
washer.  The other processor is a Tissue-Tek II (c. 1983).  I think I need a 
optoelectronic switch.

[Histonet] Gonadotrophin releasing hormone

Hi everyone-
 
Would you help me find a new vendor for this antibody:
 
Gonadotrophin-Releasing Hormone Receptor, Moust monoclonal.
 
It was vended by Vector Labs out of Burlingame CA as VP-G811.  
 
We're in the MIDDLE of a research study and having to change antibodies is 
going to be tough!
 
The original vendor said it was discontinues with no further information.  
Usually vendors can tell us why and I can't dig up anything more on 
this--hoping maybe they lost a contract or something where we can find it 
elsewhere...any help is VERY much appreciated.

[Histonet] cost per HE

Depending on how I dice up labor and materials, I come up with a cost per slide 
(HE), from receipt of specimen to the slide in the Path's hands ranging 
everywhere from $6 to $9/ slide. 
 
HELP?  How do you distill your cost per slide??

[Histonet] request for a little bit of Zinc Formalin

Hi everybody-
 
We're testing a way to improve some of our IHC staining by inserting a Zinc 
Formalin post-fix step but we can't find just a little bit!  Does anyone have 
ZN formalin where they could send us a little bit--even just 10 ml so I don't 
have to buy a huge bottle we may never use?

We're seeking a experienced grossing Histotech for a group of small biopsy 
clinics.  Must have gross experience.  The pay is GREAT and the work

[Histonet] Light green for PAS

I agree that the light green is dead and you should make some new or buy a 
commercially made solution.  However, if you want to try a different 
counterstain try a light hematoylin (usually used for glycogen) or metanil 
yellow.  Either will give good contrast to the pink of the PAS.

May we assume you've confirmed this is happening at embedding and have 
ruled out any floaters happening during cutting?
 
When embedding, keep Kimwipes or other tissues around, keep the wells closed 
and only pull out enough cassettes that you can keeep clean and clear of your 
working area.  Wipe forceps and surfaces between blocks containing fragmented 
or friable tissue, don't put forceps back in the wells without wiping. You can 
stack a few guaze pads on top of the spout to wipe as you replace the forceps 
and change the pads frequently.   Always, always only open one cassettte at a 
time and never leave the station with an open cassette on the station.  Finish 
before standing or recap and replace.  
 
If you are working with currettes, cell blocks, or other cellular, friable 
tissue, open and unwrap on a clean surface (hot or cold - embedding station 
surface or wipe or l'absorb) and don't reuse the surface before wiping or 
replacing.  If you're using knives or scalpels to scrape, make sure the handles 
and connection points aren't harboring residual tissue. Buy those little 
seamless paring knive from the dollar store--they fit in the wells and wipe 
easily.  Use swabs between embedding sessions or between people trading 
places to clean the wells and then clean them again at the end of the embedding 
session.  
 
If you keep molten paraffin in the hold bins, filter or replace frequently and 
do not reuse.  If you keep the hold bins dry, clean routinely (daily)  Clean 
your molten wax chamber periodically to remove contaminants and keep the filter 
from clogging over time. Most embedding station mfc don't condone running 
xylene through the tubes  pumps--clean hot wax will do the job.
 
We always make it the responsibility of each person to clean both at the end of 
embedding AND to clean again before starting to fully assure the wells and 
surfaces were clear and eliminate a possible cross if one person in the chain 
forgets...double system processes like you double check specimen IDs. 
 
You'll go through a whole bunch of kimwipes--but they are much cheaper than 
gauze and SOOO much better than a cross contamination situation.  IF it still 
happens, it's time to track who embeds each block to see if there's a pattern 
by person.  The point is not to write people up but to support developing clean 
habits and to adjust their habits to do it to their best ability.

[Histonet] Sweetheart of a day job - Arizona

Hi Guys-
 
I'm SO excited!  One of the places I temped and trained a bench tech oh-so-many 
years ago is looking to hire another Histotech.  They're growing leaps and 
bounds.  The Pathologist is the same I worked with--nice guy, good at his 
job--and the community is awesome.  
 
They're looking for a registered or eligible tech with at least one year for a 
M-F day shift. It's a hospital--one of those places you can stay forever and be 
happy in your job. The pay is good, relocation assistance is available.  I'd be 
delighted to help them find their 'right' fit.
