We are in the process of re-examining what we discuss in our daily huddles with 
a view to involve all pathologists, and was wondering what others are doing in 
relation to topics for daily study and as part of a daily management system 
e.g.  Metrics and Goals;  Daily Readiness Assessment -MESA (Methods, Equipment, 
Supplies, Associates); Problem Management - ownership, expectations, 
countermeasures.

Do pathologists get actively involved in huddles in your facilities?

Any suggestions for getting all staff fully engaged?

I need some help getting statistics for benchmarking for staffing evaluation

What is your annual volume of cases/blocks?
Continual flow processing?
# of IHC/Special stains
EM?
Shifts?
Days only?
Weekend coverage?
How many HTs do you have?
How many PAs?
Lab Assistants?
How many pathologists?
Do you perform any research?
What do you do regarding transcription? # of transcriptionists, outsource, 
voice recognition?

Have to disagree with performing H.pylori testing on all gastric biopsies. It 
cannot be justified as best practice if there is no clinical or pathological 
indication.

Rachel; if you are talking about testing for H. Pylori on gastric samples it 
can be justified as a best practice. I was surprised by how many kids have that 
bacterium when I moved down to Corpus to work in a pediatric hospital. I do 
miss H-town and it's theater scene.

[Histonet] hepatocellular carcinoma

As I didn't get any bites yesterday, let me try another approach.

Can anyone help with a couple of unstained sections from hepatocellular 
carcinoma?

We are investigating an antibody that we hope will help in differentiating 
hepatoblastoma (a rare malignant tumor of the liver in children) from 
hepatocellular carcinoma. We have more than enough hepatoblastoma cases, but 
few HCC

At previous place of employment we used Statlab's tape - absolutely no problems 
with stain bleeding or fading, bubbles or peeling of the tape. There were no 
problems with the coverslipper either.

Of course Sakura is going to warn against using any other product but theirs. 
Consumables are how they stay in business.

Sakura's. It is not only much better but also will allow the coverslipper to

[Histonet] Lynch syndrome

I know it is a long-shot, but does anyone know of a TMA made with known Lynch 
Syndrome cases?

We have been asked to validate MSI antibodies for evaluating CMMRD 
(Constitutional Mismatch Repair Deficiency) in pediatric brain tumors and don't 
have access to Lynch Syndrome in our pediatric facility

being a peds hospital, we are somewhat similar to Tony in Australia. If the 
clinician needs the results that day, that's what he gets (they don't abuse 
this either!). If on a Friday (or preceding a holiday), and they do not need 
the result that same day, we will process overnight and someone will come in 
and cut and stain the bx the following day.



To date, we have not had to come in on a Sunday, but someone is on-call for 
this (and other emergencies).



We do not use microwaves, but have great success using the Leica Peloris on a 1 
hour cycle, after a minimum of 30 minutes fixation.



We will not rush the special stains or IHC (C3d and C4d), but our regular 
cut-off time for those are 3:00pm for same day TAT and 4:30 for overnight IHC

Perhaps I'm missing something here too. Did you decalcify the tonsil for the 
same length of time as the mouse femur? If you are looking at  e.g. CD markers 
in bone or indeed bone marrow, it is important that you have positive control 
material that you know works with undecalcified tissue and a corresponding 
piece of tissue that is treated in the same way as your test sample (i.e. 2 
tonsil samples from the same case, 1 of which has been decalcified and the 
other not, in the same control block - we also have bone marrow control blocks 
that we do the same with)

It is too much of a generalization that EDTA preserves all antigen sites. Some 
are affected much more than others. 
So to be able to help at all, what specific markers are you looking at, what 
are they raised in, and how are you detecting the antibody-antigen reaction?

Could I ask exactly what you are looking for in mouse femur and human tonsil 
the tonsil  I assume is a control?  For what?  Whatever you are looking for is 
obviously in human tonsil.  Are you positive it is in murine femur.  I used to 
stain for 15-20 targets in EDTA decaled mouse femurs but my control was also a

Hello fellow histonetters, 

Can someone who has worked with decalcifyied bone tell me if EDTA interferes 
with IHC staining? I was under the impression it did not but cannot get 
staining in mouse femurs that I have decaled in EDTA. I have used both the 
steamer and the decloaker for retrieval and in the human tonsil the stain is 
great. 

Any suggestions..

surely not 160 characters per hour; perhaps 160 lines/hour? This is one of the 
measurements of medical transcription productivity standards, typically rates 
fall within the 100-200 lines/hour, based on 65 characters/line, and depending 
on the speciality, type of facility and the dictator! 
The other major standard being Accuracy with a goal of ~98%.
My pet hamster could do more than 160 characters per hour and he's arthritic 
(accuracy probably down in the 0.98% range though)!

Subject: [Histonet] Transcriptionist Productivity (Willis, Donna G.)

We have two types of transcriptionists working for our Market Pathology 
departments.  There are onsite transcriptionists that transcribe and have many 
other duties such as doing send outs and answering phones.  Because they have 
other miscellaneous duties, coming up with a standard was difficult, but we did 
do it.  We compare their characters typed by hour worked.  I add up all the 
character totals and then divide it between the number of transcriptionists.  
This is the average.  Each transcriptionist must be doing at least 80% of the 
average.  This helps to make sure that one transcriptionist is not doing less 
than others.

We also have transcriptionists that do transcription from home.  Since they do 
not have the interruptions or other duties, they have stricter guidelines.  The 
minimum that they are supposed to complete is 160 characters per hour worked 
however they often do much more.

I hope this helps, it's not an easy thing to keep track of, I know.  Let me 
know if you need any more information.

Many people are working on the Enzo probes just now. Mayo does have the probes 
up and running on the Bonds, but are keeping tight-lipped about their protocols.
We tried the Dako probes but never could get consistent results.

Never had any such problems with either the Max or the Bond IIIs in the 6+ 
years we have had them(no matter how many labels are on the slide - we have had 
three labels on a slide with no problems at all), unless of course your tissue 
extends to the rim of the covertile - then what would you expect?
You certainly do not need to have all 10 spaces filled on the tray. 
Suggest you contact Leica service as it seems as though your machine needs 
calibrated and realigned

We have a Bond Max and run mainly ER/PR and ISH stains on that.  After Leica 
loaded in a new version of software in October we started having problems like 
that too.  Seems you have to have all 10 spots on the tray filled with a slide 
- doesn't have to be programed, just a slide and cover tile for the weight.  We 
also were doing a workaround with a Vantage label under the Leica label and 
found out that the two label thickness interferes with the movement of the 
cover tile (sometimes...but not always), which was affecting our staining.  
Even a regular slide label under the Leica label will cause a problem.  We 
finally have gone to entering a case, printing the labels but not putting them 
on the slides, cutting the sections and then putting the labels on the slide 
for positive patient identification.  Not ideal, and I worry a lot about case 
mix ups, but that is what is working to get consistent staining here.  As they 
say, The Enemy of Good is Better.  Wish Leica had not made it better!  It was 
working very nicely before.

We here in wellington Hospital New Zealand having some problem with the
Her-2 ( Lieca, NCL-L-CBE-356 )
 
Top half of the slide ( control or Test tissue) are strongly positive but if we 
put the control tissue on the bottom half it is week to negative!
 
Does any one has the same problems with your Her-2, it is done on a Leica Bond 
(Max) III.
 
Much appreciated for any suggestions, also from Leica Technical specialists.

back in the days when I worked exclusively in neuropathology in Glasgow, the 
tinctorial stains of choice for pituitary exams were PAS/OG and OFG (orange 
fuchsin green) methods.  I don't have the references to hand , although I 
believe the first was attributed to Tony Pearse, but can probably pull up the 
methods if needed

obvious disadvantage is you need a fluorescence microscope, but then you need 
polarizing filters for Congo red (not as expensive though!)



I haven't used Thioflavine S but Thioflavine T. My understanding is that 
Thioflavine S, which is a chemically different dye, tends to fluoresce strongly 
but nonselectively, and can overstain the whole section. Either way, the 
staining is not light-fast!



Sections need to be kept in the dark until examined and usually within no more 
than a couple of days.



I used it a lot back home where it seemed to be preferred more than Congo red. 
It is a very simple stain to perform, and obviously can be screened quickly. 
Dissolving the dye in 0.1M HCl gives greater selectivity. The dye soluton has a 
shelf-life of a month or more if kept in the dark.



It is important to counterstain the sections with Mayer's hematoxylin as this 
quenches any nuclear fluorescence.



Elastic fibers, fibrinoid, mast cell granules and Paneth cell granules 
fluoresce with this method but they can be easily identified as such and ignored



The method of Burns et al gives probably the most consistent results: Burns J, 
Pennock CA, Stoward PJ. The specificity of staining of amyloid deposits with 
Thioflavine T. J Pathol Bacteriol 1967; 94: 337-344



Intersting comments  in Puchtler's paper: Puchtler H, Sweat Waldrop F, Meloan 
S. Application of thiazole dyes to amyloid under conditions of direct  cotton 
dyeing: correlation of histochemical and chemical data.  Histochemistry 1983; 
77: 431-445

[Histonet] FISH baking and dewaxing

Can someone please explain why paraffin-embedded slides for FISH need to be 
baked for so long and have extensive dewaxing in xylenes and subsequent alcohol 
pretreatment?
Our Molecular group bake for 1 hr at 65C, and then treat sections in 3 changes 
of xylene 15 min each, and 100% ethanol 15 minutes each x2 before air-drying. I 
have asked them why so long and get the standard response -- that's the way we 
always do it!

don't understand why you would buy the antibody and then ask the question. The 
spec sheet indicates that the antibody has only been shown to work on WB, IP, 
IF and ELISA. There is no indication as to whether it will work on IHC.

Luxol fast blue staining is dependent on the polarity of the solvent. Weak 
staining can occur if the solvent becomes too polar, i.e. if sections are taken 
down to water rather than 95% alcohol prior to putting in the staining 
solution. Also check the amount of water in the ethanol or methanol used to 
make up the LFB

Lately, I have had problems with my Luxol Fast Blue myelin stain not being 
nearly as dark as it should.
I am having my oven checked out  because  I have had problems with  it holding 
a  high enough temperature (at least 60C).
This seems to one factor in getting the stain to take.
But I suspect there may be other factors involved.
Has anyone had this sort of problem before? What did you find was the cause and 
how did you fix the problem?

Pathology Labs, as in the strictest sense (in most cases) results are not being 
generated by the technical staff?

There is dispute between pathologists, techs and administrators about what

What buffer system are you using? phosphate or cacodylate (or something else)?

[Histonet] RE: Using methyl benzoate on a VIP 1000

not familiar with its use nowadays, but I remember it being combined with 
celloidin (methyl benzoate celloidin) as part of a double-embedding protocol 
used for autopsy tissues. Personally I have not heard of it being used alone. 

Can still remember floating in the room where we had all our open processors 
with chloroform, methyl benzoate, alcohols, xylene, celloidin and ether in the 
atmosphere. Oh happy days - we were still high when we got to the pub after 
work!

[Histonet] fresh specimens after hours

for facilities that are NOT staffed 24/7, how are fresh specimens handled after 
hours?

Up till now, OR staff have called the pathologist on-call to determine how best 
to handle the specimen, but that process has broken down on the OR side of 
things. The OR Director does not want 2 different ways of dealing with fresh 
specimens and they now want a process that will be the same 24/7.

As we will not be staffing the lab 24/7 on the odd chance a fresh specimen 
might come, I am interested in what other facilities may be doing.

to work on some mummified tissue, with surprising success, when I was back in

I have used this reprocessing schedule from Lee Luna's book Histopathologic 
Methods and Color Atlas of Specials Stains and Tissue Artifact. This procedure 
has worked for me on a couple of occasions, You may need to change the times a 
little since your tissue samples are so small. Luna's book has a lot of 
information that is very helpful, and would be a good reference book to have in 
the lab. Hope this helps you out, give it a try.

Tissue Softening Solution
The following solution is used for softening dried tissue specimens prior to 
reprocessing or processing

  Formol-Sodium Acetate (Stock) Solution
Concentrated formalin (37%) solution10 ml
Sodium acetate 2 gm
Tap water 90 ml

 Formol-Glycerol (Working) Solution
Formalin-sodium acetate (stock)  90 ml
Glycerol (glycerin)  10 ml
 
   Reprocessing Schedule
Melt the paraffin blocks down
Place the tissue in 3 changes of xylene for 1 hour each Place in 100% alcohol, 
2 changes for 1 hour each Place in 95% alcohol, 2 changes for 1 hour each Place 
under running tap water for 20 minutes Place in formol-glycerol working 
solution until tissue becomes soft (In most instances this will occur within 
5-8 hours: extended exposure to the formol-gycerol will not harm tissue 
specimens) Place the tissue in the process and proceed with routine Processing 
schedule. How ever it is not necessary for the tissue to go through the 
fixative stations on the processor

My tissue processor was loaded and although the technician thought they started 
it, the next morning the tissues were found dry in the retort with the 
processor not started.  Another technician just started the machine so the 
tissues got processed as they would have been had it started the day before.  
The tissues look terrible, they shrank so much that the pieces started coming 
out of the cassettes even though they were plenty large enough in the first 
place.  The tissues were found to be very small and hard but were embedded, 
sectioned and stained anyway (I was not informed about this until I started 
questioning why the tissues looked so bad).



Of course the investigator is very upset that their tissues are ruined.
Does anyone know if there is any way to try and recovery these tissues?
They were mouse lungs and livers.  The livers are not as bad as the lungs, the 
lungs were inflated and they look compressed and unrecognizable as lung tissue. 
 If the investigator will ever speak to me again I am going to ask for the 
blocks back and try melting them down and perhaps try to rehydrate them somehow 
for reprocessing.

I believe Celestine Blue was used much more frequently in the UK in the 70s and 
80s, preferred to an iron hematoxylin, in many trichrome stains including 
Lendrum's MSB, as Peggy mentions; the nuclear staining withstanding further 
differentiation during the subsequent staining from either picric or 
phosphotungstic/phosphomolybdic acids.
It is a difficult stain to prepare as it has to be boiled with glycerin in it 
and requires glass beads being added to prevent it foaming up and going all 
over the bench (don't have the exact recipe to hand, but do remember having to 
frequently clean up the mess during my training!) We used it extensively, and I 
do not remember it having a particularly short shelf-life - but it did have to 
be filtered regularly.

backup or substitute for hematoxylin, particularly in the old outmoded Pearse 
stain for pituitary cells. R.D. Lillie as I remember didn't think much of the 
dye, and I don't think this dye is a very good topic for a study such as the 
one you describe.

There must be some way for the Fire Department to be able to access MSDS. As 
Victor rightly points out, if your computer system is down, that option no 
longer exists. It is not only prudent, it is mandatory for the Fire Service to 
be able to look up any chemical within your facility.

If you cannot satisfy your local fire daprtment, they will close you down 
quicker than CAP or CLIA ever could.

Using frozen sections from human muscle biopsies, we assessed the value of Nile 
blue and Nile red, two fluorescent probes, as stains for lipid droplets in 
normal and pathological skeletal muscle fibers. In normal muscle, lipid storage 
disorders, and mitochondrial myopathies, Nile blue stained the lipid droplets 
as yellow-gold fluorescent structures. The lipid droplets were also seen as 
yellow-gold fluorescent structures in Nile red-stained sections, but the 
outstanding feature in these preparations was the staining of the membrane 
network of the muscle fibers and membrane proliferations in pathological muscle 
as red-orange fluorescent structures. These results suggest that both Nile blue 
and Nile red stains are useful for visualization of lipid droplets and membrane 
proliferations in pathological muscle biopsies.

[Histonet] C4d nuclear staining?

Has anyone ever noticed any nuclear staining with C4d in a renal biopsy? The 
control stains great, but the patient's tissue exhibits some nuclear staining 
in what look to be inflammatory cells. There has been a report of granular and 
some nuclear staining in lung transplants on structures beyond the vasculature 
but we haven't seen anything like this in a case of renal rejection.

If you are talking about frozen sections of fixed tissue, they do not adhere 
well to slides (even charged slides) no matter how thick or thin. I'm not sure 
why you are treating the sections with alcohol chloroform either. That is 
pretty harsh treatment and totally unnecessary for Nissl staining. 

Why not stain the sections free-floating and then mount them from xylene?

Hi Susan, I would suggest cutting lots thinner sections. The plus slides seem 
to have trouble holding on to very thick sections. Another possibility is too 
much glacial acetic acid in your working solution of cresyl-too much acid can

violet.  These are fixed frozen sections, 50 um, dried overnight on plus 
slides, overnight in 1 to 1 alcohol/chloroform, then rehydrated through 100, 95 
ETOHs then to water.  Into the cresyl violet solution, then 100% to 95% ETOH.  
Is it the water?  Is it the slides?  Any suggestions?

If you modify any part of the test protocol from the FDA approved methodology 
in any way, then it is no longer an FDA approved test.

Does that mean you cannot use it on your system? Absolutely not; you would have 
to validate it as you would any other antibody. 

However, you do need to be cognisant of the fact that if the FDA approved test

In the past the FDA approval has been tied to running the antibody on the 
specific vendors' instrumentation using a predetermined protocol. This was true 
for Dako's EGFR that was FDA approved.

Ventana antibodies can be dispensed out of their vials and used on any 
instrumentation, however, you may have an IVD antibody at that point.

Definitely check around and see what others have to say.

Ventana just received FDA approval for the first fully automated diagnostic 
assay for HER2 gene status determination in breast cancer patients.

I do not have a Ventana immunostainer and from what I have been told, it is a 
closed system and this new probe cocktail could not be used on another stainer, 
such as the Dako.

Is anyone out there in HistoLand using this new cocktail and am I correct that 
it would not work using the Dako Autostainer.

[Histonet] Ki-67/PHH3 double staining

Has anyone had success double staining these two antibodies for IHC? Ki-67 
tends to mask most of the PHH3 staining irrespective of which antibody we use 
first. Individually, both antibodies have clean and crisp localization. I have 
seen pictures in articles and on-line of results but am far from attaining 
anything like these.
We are using the Refine DAB and AP Red kits from Leica on the Bonds. Ki-67 is 
from Leica and PHH3 from Cell Marque

Any help or suggestions welcome

Nikon is expensive, but they are very friendly and will fix little things
for you for free (not that they'll tell you that upfront).  Our Nikon
representative is always coming by if we have even the slightest problem and
is willing to figure out weird problems that come up.  Also, they always
check the alignment on our FL bulb because I really hate doing it and I can
never get it right (though it seems so damn easy when the guy is showing me
how to do it!).
We have a Nikon E600 that is decades old and still in perfect condition.
This scope is actually featured on the microscope museum page Nikon runs, I
was very surprised to see it there!
You should try talking to the representative to see if you can get some sort
of deal.  They are very friendly and very willing to work with you.  I would
tell you to talk to Adam Henry but he works for Pennsylvania only.  He's
excellent!
You might be able to get something used from them, you never know.
Personally, I'd pay for something new, no matter how expensive (of course,
who isn't working under a grant right now? me! so I shouldn't really talk
about spending loads of money!).  Nikon is a good brand and definitely worth
the cost.
Also, pay for the APO objectives, it's worth it.  You can tell the
difference.  I have no idea what APO stands for, but they work better than
regular ones.

This is mostly for the girls - I dropped into the Ulta store yesterday to pick 
up a few things and the sales person wanted me to try this somewhat new product 
by Benefit. It was a lip and cheek tint - came in a little bottle - looked like 
mucicarmine working solution. I looked at the box that it came in and sure 
enough, one of the first ingredients was CARMINE! So I had to relate a few of 
the stories about beetles and Carmine and pirates that I learned from reading 
A Perfect Red recommended by Dick Dapson...wonder if I bored her? Maybe she 
won't bother me the next time I'm rushing through the store trying to get my 
stuff and get out of there.

I am using the Azure B as a counter-stain for melanin preparing solution by the 
protocol below. My questions are:

1. Is this solution should be fresh prepared every day?

2. How long I can store and use this solution?

3. Where this solution should be stored in the room or in the refrigerator?



My protocol is:

Counterstain Method

  The use of Azure B as a counter-stain will stain melanin a blue green 
color.

Azure B working reagent:

4mL of a 25mg/mL aqueous solution of Azure B

3.4mL of 0.1M acetic acid

600uL of 0.1M sodium acetate

27mL of DI water

5mL acetone



Incubate for up to 30 minutes, followed by a 20-second 
hematoxylin counter-stain.  The use of either counterstain should give you good 
cytological detail, with blue-green melanin and brown DAB specific signal.

(cat # DS390, Leica) for manual IHC. I've seen some beautiful staining,
the technical specialist at Leica, however, told me it is only for
Leica automatic stainers. It is an AP stain. 
Has anybody tried it?

Michel,

Most studies involving morphometric studies of peripheral nerves involve 
thick plastic sections stained with toluidine blue. In my opinion, you are 
unlikely to get consistent and acceptable staining results on paraffin sections 
for the simple reason that the intense blue staining on thick EM sections is 
more from the osmium tetroxide than the toluidine blue per se.

total outer diameter) - rat sciatic nerve - at least 500 myelinated fibers per 
nerve will be evaluated with a standard imaging soflware.

Do you think that we can perform this work with thin paraffin slides - 1.5µm 
(stained with toluidine blue), in spite of the best practice seems to prepare 
semithin sections from epoxy resin blocks.

We have used the C3d from Cell Marque for quite some time on our Bonds with 
very clean crisp results on all our transplant biopsies. The jury is still out 
on how much more information we get in addition to C4d.

My hospital has recently started performing heart transplants and we are now 
receiving numerous myocardial biopsies to check for rejection. One of our 
forensic pathologists is an expert in cardiovascular pathology and has read 
many biopsies over the years for other hospitals. His protocol for cutting 
these biopsies is very labor intensive (10 slides with 8 serial sections on 
each slide, some stained with HE others kept unstained and 2 for C4d immunos). 
 I'm curious to know what other labs are doing for these biopsies in hopes of 
getting this protocol changes.

Pam, 
We have been performing C4d by IHC for about 3 years now and C3d for many 
months on all our transplant biopsies. Obviously, being able to do this on 
paraffin obviates the need to have additional tissue for frozens (although I do 
understand the need for additional IF on kidney biopsies - but we're working on 
that!). 
Currently we get both antibodies from Cell Marque and run them on our Bonds

We've been repeatedly told by Compliance that if a pathologist (or a clinician) 
orders a test, the test result must be included in the report and the test must 
be billed for; even if the result is non-contributory.

Hey Joyce,

My understanding has always been that if the pathologist specifically
mentions the stain in the report, then it's OK to charge all the fees.
However, if the pathologist chooses to leave any mention of the stain/immuno
in the report, then you can't bill for it.  So if the pathologist mentions a
specific stain was done, but that the findings were negative, you can still
charge.  In the case of an exhausted tumor, I think the ultimate discretion
comes to whether or not the pathologist chooses to mention it in the
report.  That's just from my experience, but I would love to hear how others
are handling instances such as these, too!
