This position will perform tests and procedures in our histology laboratory;  
perform functions associated with obtaining, receiving and processing of 
surgical pathology specimens in patients of all ages, to maintain laboratory

Qualifications: Histotechnician board certification, ASCP or equivalent 
certification preferred. Two (2) years of Histology work experience within the 
last five (5) years may substitute for the educational qualifications.

Education: Completion of an approved histology program and clinical histology 
internship or completion of an accredited two year laboratory technician 
training program in a vocational school or junior college.

[Histonet] paraffin and Saturday work

We use Leica infiltration paraffin for the processors and their Blue Ribbon for 
embedding

We work Saturdays to lighten the workload on Mondays or it is overwhelming.  We 
are a 24/6 lab (no techs in lab Saturday after 3:30and until 0400 Mondays)

[Histonet] recycled formalin

How does everyone handle the use of recycled formalin in regard to tissue 
processors?  OK to use on the processors?  We have just recently installed the 
Creative Waste formalin recycler and I am looking for guidelines from those who 
have experience in this area.

This position will oversee the technical component of a work team in the 
histology laboratory including; implementation of methodologies, supply 
ordering and inventory management, interpretation of laboratory results for

supervision of testing personnel; development and implementation of training 
and competency programs for histotechnicians and histology students interns, 
and to perform related duties as assigned.
Qualifications:
Graduation from an accredited college or university with an Associates (AA) or 
bachelor's degree in medical technology, histology, biology or related field; 
plus specialized training in laboratory science. A master's degree in related 
field may substitute for the above educational and registration requirements. 
Bachelor's degree preferred.
At least four (4) years of experience as a Histotechnician or Histotechnologist 
or related field with at least two (2) years of experience in the specialty 
occurring within the last four years.

[Histonet] questions

I have a couple of questions to ask where there is no right or wrong answer, 
just curious as to the process that other labs use.
1.  After processing, how do you determine the order in which to cut and stain 
the blocks..numerical or priority driven?  2. Do you adhere to the 6-72 hours 
of fixation for breasts or make certain all breast tissue is fixed for a 
minimum time of, say, 24 hours but, of course no longer than 72?

I appreciate your responses and thanks for your time!!  I am retiring at the 
end of this year and trying to optimize some processes beforehand:).

[Histonet] Prisma stainer

We have had the Prisma stainer for 2 years and LOVE it.  Have never had a 
breakdown and it is a workhorse.  It is user friendly and we do not have to use 
proprietary reagents, our choice!

I've see that Dako's DAB Away is composed of 1% Oxalic acid in water instead 
other DAB Aways (like the one you are using) are composed of sulfuric acid 10% 
in water.

We print a staining report from our IHC and/or special stain equipment and the 
tech initials he/she checked control and the pathologist initials it OK and 
returns the form of which we keep for 2 years.  If it is a manual stain, we 
have a form we made to fill out and do the same with.  For us, it was an area 
we were sited for several years ago at inspection and now cover it well.

[Histonet] Congo red stain

We have been experiencing major difficulty with our Congo Red stain.  We used 
to do it on the Artisan, but they changed raw products which has altered the 
stain and is not acceptable by our hematopathologists.  Now we are back to 
manual and cannot seem to get it consistently acceptable.

I would appreciate any ideas from the experts out there and if you want to send 
me offline your procedure, would be appreciated.  Also, does anyone use a 
negative control , such as a keloid, on the slide?

decontamination?  Specifically, is it accepted as a means of decontamination by

[Histonet] Need your expertise

We will finally be going green by recycling formalin.  I would appreciate any 
advice or opinions on true recycling VS. a filtering type of system. As we all 
know, experience with a system or product is the best way to gain true 
knowledge!

Also, I would appreciate knowing how others are handling extra levels of 
prostate needle bx's.  We currently place them on charged slides and keep until 
the case is signed out, but it seems there should be a better way of handling 
this due to storage of the slides not to mention the extra cost involved! I 
know this is becoming more of a needed process with all of the RNA/DNA testing 
being done on diagnosed cancers.

As always, I do appreciate any and all comments, advice, or words of wisdom!!

[Histonet] used xylene/VIP 6

We have the VIP 6 and really like it..user friendly and faster TAT.

We have been having rotary valve concerns within our Leica Peloris tissue 
processor and the technician as well as Leica technical services both feel it 
is because we have been using recycled xylene in the cleaning cycle.  I was 
always told that when recycling xylene, one gets a pure product back, which I 
always found hard to believe.  We do use recycled xylene in routine tissue 
processing, rotating between new and recycled as we change the reagents and 
rotate.

[Histonet] thyroid processing

Does anyone have some good tips to processing thyroid tissue from thyroidectomy 
specimens?  There seems to be a fine line between underprocessing and 
overprocessing where the tissue becomes brittle.  We currently process them on 
an overnite run where the tissue is in all reagents for 45 minutes.  We have 
also tried a shorter run on the Peloris that takes about 6 hours.  Would like 
to know what others are doing for more optimal tissue processing!!

[Histonet] question(s)

How does everyone handle storing extra paraffin sections that are cut as a 
standard on certain protocols, such as prostate needle bx's?  We currently 
place them on a slide and save until the case is signed out.  I am concerned 
with the amount of waste and cost with the way we are doing it and would like 
other ideas.  If you keep the ribbons laid out on paper, do you stack the 
sheets of paper and is that problematic when it comes time to need that certain 
cut for IHC?

Also, please let me know of those who have the VIP 6, how you like it, pros and 
cons and also the Leica ASP300S.

What criteria does everyone use to hire for working in your IHC department?  I 
am looking for feedback as to what qualifications are expected for IHC techs as 
well as qualifications and expectations of the lead in IHC.  Also, what title

We make up a marking Eosin that we use on small bx before we place them in 
cassettes.

We do not use actual teabags, but the mesh bags from Thermo Fisher that are not 
hard to pull apart and we never have anything sticking I the corners.  I do 
know what you are referring to as some of the mesh bags ot teabags on the 
market are not mad of as fine material and are hard to pull apart and allow for 
specimens to gather in the corners. Our mesh bags are used for all specimens 
the PA's can pour into the bags.

We also use the Obex papers for needle bx type specimens that can be laid out 
nicely on the paper and folded over the tissue once for optimal processing.

On tissue processors, we are new this year to the Peloris processor from Leica 
and love the versatility of two retorts and the great reagent management which 
allows for less waste and less maintenance tech time. I was always a Sakura VIP 
fan previous, still am, but cannot compare the two retort option and how LEAN 
it has mad our lab!

I agree with Rene Busa on his assessment that thawing and refreezing is very 
very bad for your epitopes. You don't do it with antibodies, you don't want to 
do it with your antigens either. Keep the slides you will not need frozen, 
taking out only what will be needed. Work quickly as exposure to the air will 
start the condensation process.

I also agree that there does not need to be an ethanol rehydration step. That 
might be useful as a permeabilization step, but you only need to hydrate your 
tissue with buffer. You might not have realized that alcohol can permeabilize 
cells (even though they are already exposed through sectioning) and also affect 
the protein folding, so if your antibodies are already working using this 
scheme you might see a difference if you quit doing it. You also might not, so 
it could be useful to test it.

As to whether to use heat to thaw, you can try putting the slides in front of 
fans at room temperature, or you can try fan forced ovens set at 25 or even 37 
degrees if you are worried about heat. I have worked with a researcher who used 
unfixed cryosections of brain and put them in a 95 degree oven for 2 minutes 
prior to freezer storage. They were still able to get antibody and mRNA 
staining from their sample. I have also read accounts of people using a hair 
dryer (blow dryer) on the heat setting on them as well with no ill effect on 
their published studies. Who is to

expect in deeper/recut requests?  I am trying to establish if our pathologists 
order more than the norm or if I have a quality issue unfolding!!

[Histonet] microtomy

We have run into an interesting scenario and wondering what the experts 
think!  We cut bone marrow bx's and lymph nodes for lymphoma @ 3 microns on one 
particular microtome.  Within the past month, the hematopathologist has felt 
the sections are thicker than the usual 3 microns.  I had our service 
technician  measure the microns and the equipment was cutting as set.  I had 
the blocks cut on a different microtome and we have seen variations there also. 
 My question is, does the amount of time on ice make a minor difference in the 
section thickness?  I know a lot of responses may be the difference in the tech 
cutting inasmuch as how fast they turn the rotations, etc., but,we have ruled 
out that variable by having more than one tech cut at the microtome in 
question. I am stymied as to how to remedy this fluctuation!  This is why we 
love histology, so many variables to create a problem and why I love histonet, 
so many techs to help one through a dilemma!!  Thank you!!

For those of you who use reagent alcohol, have you ever experienced any 
problems in processing or staining, such as artifacts, crystals forming, etc??

How long are unstained slides usable?  Do any of you pick up extra sections 
from a ribbon if the tissue is minimal ?  We do and have used them at times, 
but a pathologist would like them saved forever in case tissue is needed for 
a molecular test and there is not enough left in the block.

[Histonet] NCCI policy update

Is everyone aware that beginning 1/1/12, we can no longer bill for each block 
regarding IHC billing, only one unit of billing for each part type no matter 
how many blocks are stained?  Also IHC cocktail stains, such as PIN4 must now 
be billed as one unit even though multiple antibodies are reported out.

[Histonet] gram controls

I remember giving a couple of chicken livers to micro and they incubated them 
overnight with gram +/-. I then cut those pieces smaller and processed them in 
separate cassettes.  This process has owrked real well for us.  Was in the 
Journal of Histotechnology but cannot remember which issue a few years back.

We use normal prostate and we have also used skin in the past-they both work 
well.  I would avoid pre-cutting a lot of controls, though; it has a tendency 
to loose antigenicity.

You do not get 100% alcohol back from recycling, so it cannot be used as such 
in either the processor or stainer.  Basically, you have to use it as 95%.

[Histonet] blades

Trying to clean up some things hanging out there in our lab and wondering what 
everyone does with a blade that has been used minimally and tech done for the 
day with the microtome.  Where do you store that blade for use tomorrow or do 
you toss and not worry about the cost involved?  I do not like them sitting on

[Histonet] Von kossa stain

VonKossa stain when you have no sunlight to use?  We used to use a 60 watt 
lamp, but haven't done one for years and am bringing this stain back to our

[Histonet] QC documentation

How does everyone handle their QC documentation on special stains and IHC?  We 
currently print out the run information from our stainer(s) and have the tech 
initial for her QC and the pathologist sign after they review the slides.  I am 
hoping that someone has a way of doing this electronically.  I hope to learn of

[Histonet] Microwave processing

Does anyone know what the compliance is, if any, with JACHO or CAP regarding 
the pathologist doing a paper QC on each microwave run?  We have been having 
them sign a QC sheet that we hand in with each microwave run and our looking at

[Histonet] Parathyroid staining

Does anyone use Toluidine blue to stain parathyroids when they come in fresh 
for a frozen section?  One of our pathologist heard of this and I am not 
familiar with it.  I have heard of it with the Paragon stain, but not certain 
what percentage would be needed if only Toluidine blue was used.

[Histonet] Microtome alignment

We recently purchased the new alignment tool from Source medical products that 
works on any microtome as it uses a leveling bubble.  It is round, so it makes 
certain all is level from any direction.  Works well and has helped us out.  I 
have staff use it daily as it takes minimal time.  Only $200.

[Histonet] decalcification

I would appreciate any feedback on what all are using in your decalcification 
process.  We get a lot of large bones in and the past 2-3 months have noticed a 
huge problem in our microtomy process with these samples.  We have been 
grossing the bones in and leaving the sample in the cassette in 10% formalin 
for 24 hours befoere placing in decal for up to 8 hours and still having the 
inner portion of the sample look underprocessed and crunchy!  Any suggestions 
would be appreciated!

[Histonet] ER and PR validation

In a recent article written by 4 pathologists, including Dr. Elizabeth Hammon 
and Dr. Patrick Fitzgibbons, the recommendations are now including testing with 
a lab that has validated it's assay against clinical outcomes.  How is everyone 
planning on fulfilling this requirement inasmuch as we are not aware of any lab 
in our metro area with this kind of compliance.

[Histonet] Lot numbers

Would anyone be willing to share with me how you record lot numbers of all 
reagents and histology components as they are used?  I have a couple of ideas 
on how to capture this, but, they seem too detailed!!  Obviously, our IHC and 
special stains done on the automated stainer are documented by the equipment, 
but, looking at all the hand stains, HE's, processors, etc.,etc!!

[Histonet] questions

We have been having problems with underprocessed placental membrane, some of 
which are cut fairly thick, but am seeing it on more than just the thich 
samples.  Does anyone out there in histoland have a special process for 
placentas or any helpful hints?  I do know that many times the placentas sit 
without formalin in LD for hours before they bring them to histo and add 
formalin and this seems to me it could be a factor, even though we have them 
sitting in formalin for a few hours before processing.

Also, does anyone do the high-iron diamine stain for intestinal mucin staining? 
 Do you do it with an Alcian Blue-PAS stain?

[Histonet] EDTA

addition to formic acid, has EDTA in the mix.  I have never worked with EDTA so

[Histonet] Negative control

We have not done negative controls, but, have recently started placing both a 
negative and positive control on slides for Congo Red based on recommendation 
by Mayo. We are using a keloid scar.  They state that on consult cases, they 
have often noticed overstaining on high collagen cases.  Our pathologists 
thought it would be good for comparison!  But, that is the only one for now!!

Our pathology department has been using Dragon for 3 years now and really like 
it, both PA's and pathologists alike!  It is time saving and easy to adapt to, 
even the Pathologists who have their foreign accents!!  In fact, our entire 
facility is switching to Dragon for dictation.  The only negative was the loss 
of FTE's for secretaries, as it took their work away!!

We use Copath for Histology LIS and Epic as our Hospital and clinic based 
system.

In Epic, you can retrieve who reviewed your results by looking at the result in 
Chart Review.  Look below the results, it lists who and when.  There is also an 
electronic trail that Epic can produce if needed.

[Histonet] specimens

The first problem was with the tech who removed thespecimens from the OR
without checking to make certain everything was included.  She/he should
never have left the OR without responding to the issue and that would
have taken the blame off of the histotechs.  We have OR bring specimens
to us and sign in with a label as used on the container, initial and
date/time everything.  The techs accepting specimens have to sign off
and accept responsibility at that point.  When we did go to the OR, a
few yaers ago, we had the same logbook in place there.  The OR personnel
had to sign everything in and when we picked the tissue up, we checked
each specimen off in the logbook with our initials and time/date.  Same
process, only in reverse.  Once anyone initialed the logbook, it was
that person's responsibility as to the specimen integrity.

[Histonet] Flammable storage

How does everyone store any reagents that need refrigeration that are
also flammable? We have always just stored them in the routine histology
refrigerator.  I have never had this concern come up before and the
charge tech of toxicology is now questioning this type of storage.  Any
guidance on this would be greatly appreciated!!

 assigns the surgical case number, all info is linked to that listed
 surgical accessioned number and your 2 or more identifiers are pulled
 up with the case.  That also keeps within HIPPA guidelines to have
 patient information openly available when blocks or slides are sent
 out or left out in view.  Otherwise, barcoding is the ideal way to
 place, view, and tie all information needed!!

[Histonet] Microtome alignment

We use the microtome alignment instrument, which can be purchased from
several companies.  It is in our weekly checklist that all microtomes
have to be adjusted (checked) with the microtome aligner.  All
microtomes are cut at the same angle also!

[Histonet] Control tissue

The best control is healthy skin with the connective tissue in place but a 
tonsil is also great for checking how well your lymph tissue is staining.  
Tonsil should be cut thinner just like the lymph nodes so you should use the 
tonsil when staining lymph nodes.  Make sure the tonsil has connective tissue 
in it so you can see the eosin staining in the collagen, blood vessels, 
cytoplasm, and RBC's.  Another good tissue is cervix with the glands.  The 
glandular area gives you the nice hematoxylin staining and the muscle and 
collagen fibers give you a nice contrast on the eosin staining plus you can 
always get cervix. 

I would stay away from the appendix since there is usually a lot of necrosis 
and the placenta is a little to bloody so the eosin is always some what 
overwhelming for a good evaluation.

[Histonet] Can you believe it

Has anyone ever heard of cassettes and tissue totally disintegrating in
formic acid decal solution with a high heat on the solution for about 3
hours?  We had 3 cassettes with tissue for decal placed in our
container on the platform for adding agitation.  Someone had moved the
knob to heat and this wasn't noticed until about 2-3 hours later, when
it was boiling!  We cannot locate the cassettes (I think someone
misplaced them) and others think they totally disintegrated with the
decal and heat...what does everyone think?  In all of my years of
histology, never have I heard of anything so weird ..

[Histonet] Fatty tissue

I would like to know how other labs are processing fatty tissue,
especially breast for optimal processing?  We have been doing a few
reprocessing of these specimens and my pathologist feels this should
not be happening!  Yes, the PA's are, at times, cutting the sections
larger than 3 mm thick, but, at times the tumor is ingrained in the
fatty area and they need to obtain as much of the perimeter as they can.
Does anyone agitate or heat fixate their specimens prior to processing?
Would appreciate any feedback on this age old dilemma.
Also, I only heard back from one person on the recycling issue.  Any
input would be appreciated as to savings incurred and a still vs.
recycler.

[Histonet] recycling

We reycle xylene presently and are looking into recycling alcohol and
formalin, mostly to look for a costs savings.  For those of you in
histoland that recycle those products, how generous are your cost
savings?  I am in doubt as to the fact that you cannot get 100% recycled
alcohol as a byproduct of ones' recycling.  I still will need to
purchase absolute alcohol and that is the majority of alcohol ordered.
Also, are the aldehydes and buffers costly that one needs to replenish
in recycled formalin?  And, has anyone seen any problems with their
processing or staining when using these recycled products or do you find
the same, optimal slide production??  Thanks much for sharing your
experiences!!

[Histonet] validation

Is it better to use a lab's own tissue for validation in a microarray
for Ihc purposes or would it be passable to use TMA's from another

[Histonet] Amyloid

We have been cutting our tissue for amyloid staining @ 8 microns.  One
of my pathologists heard that 10 microns is now standard  and to use a
negative and weakly positive control.  Does anyone have any new
information in this area?  Thanks ahead of time!
