If all you need is one-to-one communication (which doesn't meet your "up to 4 people" requirment) it is actually possible to dial an IP from a VOIP phone (at least some of them) and have a direct phone to phone connection. Now the phone has to be exposed on the internet, but it is not too hard to restrict where that port is accessible from. Skype is about as good as they get for VoIP applications. If Skype doesn't work well, I don't think a different VoIP app will work much better. The problem is latency on "the internet." If you know the public IP address of the endpoint (or close thereto), run a traceroute and see how many hops there are to that endpoint. Latency = messed up communications. Data applications are designed to work with latency. Voice (and video) cannot.
