I can reproduce in Trusty.

Christopher, just to be sure we are talking about the same thing: this bug is 
about rotating/moving the part, like a CAD software would do. (eg you can see 
behind the part).
If that's what you are able to do, please can you explain how you do it?

In first case, Ubiquity should at least warn the user, and recommend to
abort and install Ubuntu from a UEFI-session.

In second case, the fix you describe is not enough if Ubiquity continues to 
blindly create an ESP when detecting the UEFI-session, then grub-install will 
be mistaken by that ESP. 
Please make sure Ubiquity is fixed too.

Ubiquity creates the ESP line in /etc/fstab in a way that conflicts with
the description of the /etc/fstab in the man page for fstab.

The 16th paragraph in the man page for fstab describes fs_passno.  It is
supposed to be 1 for the root filesystem, 2 for all other filesystems
which are to have fsck run on them, and 0 for all other mounts for which
fsck is not to be run.

However, Ubiquity uses fs_passno 1 for the ESP line in fstab.

IMHO, that bug simply shows that dist-upgrade breaks the boot by blindly 
(re)installing GRUB whatever the boot setup is.
In other words, dist-upgrade does not support enough boot configurations. 

A very simple and quick fix is: dist-upgrade should not install any
bootloader.   (why taking the risk of breaking the boot?)

Plus, there are probably several sub-bugs (several types of "unsupported
configurations"  leading to the same error message) mixed in that
report, would be easier to understand if each user was creating his own
report, then devs could regroup them into categories.

please describe the procedure step-by-step and I will ask the users to
follow it.

ALL THIS BUG DESCRIPTION CONCERNS ONLY 64bit UEFI COMPUTERS
 (= most of Windows8 computers, so sadly most of recent computers)

we have 2 big issues:

1) Ubuntu-32bit ISOs cannot boot in UEFI mode (bug #102 ), so
Ubuntu32bit will always be installed in Legacy (non-UEFI) mode. Thus,
Ubuntu32bit cannot boot after installing it, except if we install an
UEFI bootloader such as grub-efi.

2) According to comment 7 of Bug #1020446 , 64-bit UEFI firmwares
generally will only load a 64 bit boot loader. This is a limitation of
the firmware.

So in most cases, installing Ubuntu-32bit fails, and there is no way to make it 
work !!!

UEFI.

2) and the CPU is 64-bit compatible, 
3) and if the install disk is 32bit. 

then currently, the Ubuntu installer will install Ubuntu-32bit in Legacy mode. 
(reminder: current Ubuntu-32bit cannot boot in UEFI mode)
However, the computer will very probably be using UEFI boot (at least for all 
preinstalled Win8 computers). Result: Ubuntu will not boot.

Fix proposal:
when detecting the 1-2-3 conditions, the installer (Ubiquity) should strongly 
recommend the user to use a 64-bit install disk.

First, a blank line pops up in the terminal, ready to type in another
command.  A second later, a window pops up stating: "System program
problem detected" and asks if I want to report the problem.  Once I
click to report the problem, it says: "Sorry, the application blkid has
closed unexpectedly."

I tried closing the terminal, opening again and trying again.  This time
I get a fresh line like before, but no error messages pop up.
Basically, nothing appeared to happen.

Rebooted and got the same results: first an error message, next nothing
appears to happen.

The 30_os-prober  and  30_uefi-firmware   are the same as from the stock grub2 
install.
Boot-Repair only adds the 25_custom entries.

The subject of this bug is that in step 2 (when SecureBoot enabled) of
this bug, the  stock grub2 entries (30_os-prober  and  30_uefi-firmware
) don't allow to boot Windows.

IMHO, this is not duplicate of bug #807801 , because:
- in #1183074 , the stock grub2 Windows entries exist, but fail only when 
SecureBoot enabled
- in  #807801 , the entries fail (or don't exist) even when SecureBoot is 
disabled.

New observation: with --error, the bug is 2x more important than with
--info , --warning.

We don't know if your issue is the current bug (Input/Output error), or Bug 
#1091477 ("Read-Only" error), or else, because you didn't mention the output of 
"grub-install --efi-directory=/boot/efi --target=x86_64-efi".

This is not a PDF, but a SO.

If printed then modified, the document is refused by the government.

This .so can be modified by Evince and Adobe Reader. The problem is that
it cannot be modified when opened inside Firefox.

https://www.formulaires.modernisation.gouv.fr/gf/cerfa_12100.do is a form from 
the French government.
When I open it normally from the government website, it opens by default in 
Firefox, displays correctly, but it cannot be modified.

To fill in it, I need to workaround by saving the file (right-click ->
save link target as) on my disk, then open it in Evince. From inside
Evince, it can be modified correctly.

+ 1) Via Gparted create another EFI partition (FAT32, 200MB, located in the 
first 100GB of the disk), move the 'boot' flag on it
+ 2) Install grub-efi in this new ESP (eg 
https://help.ubuntu.com/community/UEFI#Converting_Ubuntu_into_EFI_mode ).
+ 
+  WORKAROUND2 (to be tested):
  1) Backup the files in the ESP
  2) Format the ESP
- 3) Restore the files
- 4) Try to install again
-
