Tried upgrading to 3.2.22-030222-generic, but bug is still present.

I have a cron job that runs, twice a day, switching CCTV monitor on/off.

/usr/bin/xset -display :0.0 dpms force off
/usr/bin/xset -display :0.0 dpms force on 

This bug occurs after a few days. Rapid issuing of dpms on/off does not
appear to trigger it faster.

Just for fun I did a clean install of 12.04. Did updates then went
through the precise proposed steps again.

I didn't get the crash, but again am stuck in 640x480. Uninstalling both
nvidia-173 and nvidia-173-updates brings me back to 1024x768.

I also have installed the precise-proposed drivers in both ubuntu and
xubuntu 12.04 on the same machine (NVIDIA GeForce FX 5500) and it works
well on both installations, but in ubuntu it keeps using unity-2d instead
of unity. Is this the correct behavoir?

Addition information, I just check log and times, the report happened on
the /usr/bin/xset -display :0.0 dpms force off not the force on. Hope
that helps in fixing bug.

Andy, you describe exactly what I'm seeing and I'm on a T520 with Intel
Graphics as well.

It just happened again shortly after coming out of suspend. Kicked me
back login screen with a segfault in the X log. Same stack trace as the
first one.

I've installed xserver-xorg-core-dbg xserver-xorg-video-intel-dbg now,
hopoing it'll get me a better stack trace.

BTW amd64 is just another name for x86_64 and so doesn't necessarily
imply an AMD based machine.

I am using ubuntu 12.04 LTS
And I have an Nvidia graphic card, with a driver version 295.40
I have a new Dell Latitude laptop E6420, with a Intel® Core™ i7-2640M CPU @ 
2.80GHz × 4 processor
I have it attached to a docking station at my office with 2 Samsun monitors. 
I can get them to work using the Nvidia X server settings, but everytime I turn 
it on this error appears and I have to reconfigue it manually. I'm attaching a 
screenshot of the error message.
Hope it helps to fix the bug!

I have similar problem

There are 4 interesting symptom

1. i8042 interrupt trigged when touchpad move up/left/down/right
2. evst shows the coordination when touchpad move up/left/down/right
3. there is no event in XEV when touchpad move up/left/down/right
4. touchpad is still disabled even gnome-settings-daemon is killed

According 1 and 2 we can have conclusion that hardware interrupt and
kernel even is generated correctly but there is no any X event in
symptom 3.

Since 2.6.35 touchpad is now properly recognized as a Synaptics-type
touchpad, the X event is from xorg input synaptic driver .

there were 3 syndaemon process while touchpad is freeze, and the property 
TouchpadOff is 1

The nouveau drivers do not support my display resolution (1600x900),
thus I was unable to evaluate the bug using the open source drivers.

After re-installing the Nvidia 302.17, I now find that the problem does
not occur on the first instance of starting LibreOffice. However, the
problem does occur after closing the first instance and starting
LibreOffice again.

There was also a kernel update today, but I don't know if that made any
difference.

still present in 12.04 (64bit) with the 295.49 (X-Updates) drivers on
the GeForce 210. I don't know if it is a heat issue or if heat
generation is just a symptom of whatever issue is causing the problems.
I sure would like for the problems to go away though, as this is the
second release with the same problems using either the default or the
X-Updates drivers and both releases use the 3.x series of kernels. 11.04
is the last release that I could even think about using OpenGL in with
any sort of reliability which is very disappointing.

oh yes, update: the screen no longer goes black, but instead as Sergio
pointed out, it freezes which I suppose might sound like progress,
except that the system reboots itself after a few seconds and the
problem still renders OpenGL useless.

It looks like this bug is still around.  I was running a 12.04 guest
with the vmvga graphics, and on logging the emulation just dies.
Running 0.14.0+noroms-0ubuntu7+dnjl1~lucid0  I have done almost no
troubleshooting on this yet, and the vga driver works well enough for
now.

This bug persists in 12.04 until now. I have a ATI Mobility Radeon HD 5650 
Graphics card and am connecting my laptop display (left, 1366x768) to my 
external monitor (right, 1920x1080) with HDMI.
Luckily Andreas Gölzer's tip with xrandr is a working workaround.

Same problem around here. Disabling Sync to Vblank wont work for me.

The bug has appeared with 2 variants: After long time of inactivity
(everything freezes, except the mouse) and after suspending the notebook
by closing the lid (black screen, mouse still can be seen and moves)

I'm running an M880 RAdeon HD 6650M on ubuntu 12.04

OS still works, since CTRL+ALT+F1 works fine. However pressing
CTRL+ALT+F7 return you to the stuck screen

Accepted mesa into oneiric-proposed. The package will build now and be

a few hours, and then in the -proposed repository.

the same indications of both reproduction and anecdotal fixes are
present there. I might give the upgraded kernel a try but I loathe using
packages so far ahead of the repos and PPAs.

Lasse,

Interesting that you are on a T520 Intel graphics machine as well.  So
we clearly are having the same issue.  I have a 32 bit install to be
consistent with some other older desktop machines I support that cannot
handle 64 bit.

I got this machine new a few months ago so have only seen the problem on
12.04.

I never use my intel gpu anyway, since I have an nvidia graphics card in
a desktop pc.

So finally, having had enough of this miserable bug, i blacklisted the
intel gpu.

I blacklisted the i915 module like this, in
/etc/modprobe.d/blacklist.conf:

blacklist i915
blacklist i915_drm_kms_helper

The second line may be redundant (that module should not load without
i915).

I feel as though a great and terrible weight has been lifted.

The i915 module is broken, and I don't know how to fix it, but at least
I am free of these crashes.

This issue is still present even now. When installing 12.04 again on my
main pc, and rearranging the desktops after fulling updating my
computer, my system instantly froze again. Also, after my initial boot
up my computer would randomly flash black and then freeze shortly after
the picture reappearedSo far my experience with AMD card with Ubuntu
has been less than satisfactory. The fglrx drivers work as expected,
though I get errors when trying to install them with Jockey and had to
install it via the command line. Also, with the xorg edgers ppa and the
latest 3.5 kernel all the AMD problems go away..I very sincerely
hope that in the time the next Ubuntu release comes out (or even this
one) we can just use the now-working drivers found in the xorg-edgers,
and not break them
