AR System permissions - have you checked the whole workflow on that angle?
That's one of the things I can think of where you won't get an error if a
user does not have permissions to fire a piece of workflow that is supposed
to fire while its performing that action..

Does your Escalation show that its currently disabled?
What does your Escalation log say? Does it show any other duplicate
escalation perhaps that is running the same actions?

back of my mind is unique indexes cannot be modified in an overlay (pg. 131

You should be able to go to the AR System Administration Console just like you
did to turn on Escalation logging and go to the Configuration tab and see if
the checkbox for Disable Escalations box is checked.

You should try to figure out if calls are getting queued up before processed
in to a greater extent. An old rule of thumb from BMC says that if 10% are
queued, add more threads.
