I took a very quick look at the latest file and found many, many
incorrect x-refs to figure numbers, as well as other formatting
problems. At this point I have no hypothesis about the cause of this
continuing problem or any quick and easy way to fix it.

UPDATE to my previous note. I have now had time to check your second
file and I have found the problem.
You inserted two hyperlink entries at the beginning of the Structure
line instead of one at the beginning and one at the end. When I
corrected that (and clicked All, then OK), the TOC

The links should be there, but they probably do not show in blue as you
might expect. You’ll need to change the character style (probably Index
Link) if you want that to happen. I’ll look at the file later to see if I’m
correct.
You probably started with one of my files, which was

I followed your instructions to add the hyperlinks, then updated the
index, but it still does not appear to understand that the links are there.
I have committed the changed ODT file back to the review folder.
