I wish I had some advice for you.  I'm sure others have given you some by now though.  I don't know.  I would want him neutered.  It shouldn't take much sedation at all for a neuter.  It's not 'major' surgery like a spay.  As far as the heart murmur your vet should be able to tell you how 'bad' it
