My problem with JT65-HF is now solved, thanks for all the input!
Problem was that I tried to use DDUtil to share PTT among different
applications, and it looks like this is not supported.
Dedicated another com pair for PTT and JT65-HF, and now it works like a dream.

Something is wrong when -40 produces enough audio for overload. Could you
give us a screenshot of PowerSDR? In your PSDR setup, you don't need to
check enable VAC AND enable VAC for digital modes. The first checkbox leaves
VAC on all the time while the second checkbox turns it on only when you do
to 100%
