You don't mention the setup on the Reference Pages of your TOC file,
which is what controls the formatting of the TOC entries as far as
what variables are invoked. The other info you provided controls the
formatting of the TOC entries, but not the content of them.

The mistake you are making is trying to apply autonumbering
to the paragraphs in the TOC. The $chapnum variable can only
have one value in a given file, so is you use it in autonumbering

1) If the first paragraph in the title is centered, it will constrain the
whole title to the width of the table. You could have a tiny, blank,
centered paragraph in each title just above your normal, left-aligned

I create user guides for a software company, and we distribute these guides as
PDFs with the software downloads.
One of our products is a web portal. As part of the initial install of this web
portal, we want to include an HTML TOC to the various sections within the user
guide PDF. As

I'm using unstructured Frame 7.0 on WinXP. When I perform the
following steps, the image I imported by reference vanishes:
1) Click File > Import > File
2) Double-click an image file (BMP, PNG, etc.)
3) Set the DPI and click Set.
4) Click the image inside the anchored frame to select it.

The most common cause of mis-ordered PDF bookmarks (and
specifically chapter headings that are in the wrong place relative
to section headings) is the use of separate text frames that
are not connected in the same flow. Is your chapter title
