For the first time, I was trying out the index feature of LyX. I've marked
tens of works for inclusion, and added the index at the end of the doc.
No index appeared. I exported Latex, and ran Latex and makeindex manually.
That worked fine, and now the index also appeared from LyX

Did you try to re-install Acrobat on the Windows machine? Even if you
can open other pdfs, the executable or libraries can be damaged or
incomplete.
