Yes, that’s all there is to it with Pages. You can certainly create a
hierarchical table of contents if you set different levels of headings.
Everything you want to show up in the table of contents for each heading should
all be on one line, and if it’s too long, use a line
There is no specific command to insert headings into a Pages document. You have
to set a shortcut for each type of heading you wish to use and you can create
your own.
There are 8 available shortcuts that you can assign to paragraph or character
styles: F1 through F8.

To get access to the menus for the styles, interact with the table and find the
appropriate style, Bring the mouse, do VO-Cmd-j then interact again and do
VO-Right Arrow and you’ll be on the menu button.
Yes, it’s a bit of a faff but there’s no fancy stuff with the mouse pointer
Using the latest pages, I no longer seem to be able to get into the styles
context menu where I can assign the shortcuts. Has Apple shot itself in the
foot again with the latest release? It's been a while, but I seem to remember
it involved interacting with the actual style, and then locating

I suppose that depends on what you consider easy. Pandoc can generate EPUB 3
documents from any of the input formats it supports,
