I have a 411 that I put a new license key on so I could upgrade to 5.x.
Now when it boots the console shows:
RouterBOOT booter 2.12
RouterBoard 411
Authorization: Passed
CPU frequency: 300 MHz
Memory size: 32 MB
Press any key within 2 seconds to enter setup..
writing settings to flash...
Try NetInstall. This happened to me one months ago with two RB1100AH

As I put in the original post, it does this with Netinstall and normal boot.

Are you reformatting the drive with the netinstall?

I reformatted it from the startup menu. It does not get far enough for
NetInstall to do it.

N is funny about antenna geometry and that geometry will mess up N
something fierce. Most likely it will disable the other antenna as
disconnected. What Scott said, use a second card. Much more sane.
