I would personnaly use cocoon for its pipelines and to control the
workflow of data (step 1).
But I would use BIRT ( http://www.eclipse.org/birt/phoenix/ ) as the
rendering engine which has a great UI for designer and cover all the
format he wants (step 2).

I had the null exception too, I remember solving it by changing to a
newer version of the batik library.
I also tried using the svg syntax and the fo:instream-foreign-object element.

I did an early work on this one year ago which generated xsl-fo for
FOP 0.20, I can send the project tomorow if ur interested.
But it can only support a basic subset of what birt is capable of
(avoid scripting, cross-tables).
Birt team used to use FOP as their rendering engine (they had
into MS Word) and the image didn't appear.
What is wrong?
Does it work only when we use Cocoon or one of MS Office applications,
because they know how to put this kind of information into?
