I don't have this issue at all with my iPhone 11 and iOS 15.1. Have you checked to make sure the setting which controls this in Voiceover Settings > Audio is not turned off? Also, you can always just plug in the Earpods or use any other wired or Bluetooth headset as a work-around. -Original

Before you do that, try waking up the phone first. If the phone is asleep, it will not switch to speaker. I hit the power button once and that wakes up the phone and then it will switch to speaker. If you have a home button, try hitting that first then moving the phone away from your ear.
