What is the value for <maxcache> in config.xml for the JDBCSpoolRepository?
If I recall correctly, it defaults to 1000. I found that 250 worked better
under high load, although that likely will vary with available CPU.

If you are going to use MySQL v4, you must keep up to date. MySQL v4 is
still actively developed, and each new release is fixing dozens of bugs.
You might also try switching to Connector/J v3.0.9. We cannot distribute a
newer JDBC driver because of MySQL licensing restrictions.
Personally, we're sticking with MySQL v3 until the MySQL v4 Bug Parade slows
down a bit.
