Simply put,anything that cannot do by the regular SIP entities like UAC/UAS, 
proxy, CSCFs can be done by a B2BUA.

Here is a good use case:

When you want to hide transfers from ITSPs (many do not like REFER) you need to 
use a b2bua to keep the signaling local to the pbx that generates the REFER. 
You can convert the REFER transfer to a Re-INVITE on the ITSP side. You cannot 
do this without a B2BUA.
