sipxbridge ( and indeed all of sipx ) uses end to end signaling for
codec negotiation/renegotiation. There is no codec filtering. Your
ITSP must be able to support mid-call codec renegotiation (a
requirement of RFC 3261) for things to work correctly. You can find
out very quickly by trying to place the call on hold with MOH enabled.
Release the hold and see if that works.

If all works correctly, please share your settings so I can update the wiki.
