I tried to find the perfect combination of centralized/decentralized for a 
similar setup. I wanted centralized for the same reason you wanted. I'm in 
healthcare, and we could not have any potential issues with the remote site 
dialing out if the T1 to our main office went down. In the end, I couldn't find 
a clean way to do it without putting a remote server and FXO in. That was with 
4.0.4 though. Maybe some of the new branch features will give you some more 
options in this area.
