One thing I forgot to note is that someone was on the phone when all of the 
registered devices went away but she never lost her call.
After she hung up, she used her cell phone to call her sipx phone which is 
internal and it went directly to VM.
Without being in the list of registered phones, she is still however able to 
make calls.

As a general rule, SIP phones do not need to register in order to make outgoing 
calls.  They need to be registered at the moment they receive an incoming call. 
 But once a call is established, registration does not need to be maintained to 
continue the call.

It is likely that your dropped call problem and your failed registration 
problem come from the same fundamental problem, but the mechanism of the two 
failures is completely different, so the two problems need to be pursued 
independently.
