We use registration auth and it has been working very well.
3 things I don't like about Appia:

 * You have to buy a calling plan
 * No voip.ms like interface for your account
 * IMO, The setup process/project is unnecessary

On the other hand they have t.38 and their voip service has been rock solid.

If this is a big account then you should consider http://voipinnovations.com/ . There is a $200/month minimum. They do t.38 and their price per min is very low.
