When printing a PDF file if I hit the "scale each page to fit paper",  
it seems to shrink the image even if everything is originally well  
inside the paper (indeed, it does this even when there are 2 inches of  
margin or more on all sides).

This seems especially odd. And symptomatic of what I really care about  
which I describe next:

If I crop the file using CMD-K then do the same I can get it to grow  
the PDF image within the page, but less than I would want.

Does anyone understand what the algorithm or objective of "Scale each  
page to fit paper" is? I would like to be able to use it and have it  
not shrink something that already fits, and have it expand something  
that is cropped much more.

Is there any way to change this default behavior?

By the way, I'm aware that this is an issue that is not specific to  
Skim (it does the same in Prevew) and must be a PDFkit feature or OS X  
setting, etc. Still I thought this list was my best shot at someone  
having the knowhow have the know-how and interest in this.
