Is it possible then that the media box is larger than the page, or that it 
looks like  their algorithm is adding margins to that media box, to explain the 
shrinking that occurs for a normal Pdf?

Printing uses the media box for the PDF, it does not care (or even  
know) anything at all about the "content" of the page.

Also, for your information, printing is completely inside the realm of  
Apple's mostly private code.
