Speculating: Might it be that the media box is fitted inside the 
_printer_ margin? I.e., that the extra margin's you see is what the the 
printer driver says?

At least in Acrobat this is what happens (if you say 'Fit to page' and 
print an A4 pdf on A4 paper, you get a 96% scaling, cause it is fitted 
to the printable area - and not the page).
