The picture is worth a thousand words, see figure 8.6.5.2.1.3(B), 2013 edition. This shows pretty clearly that for a 13 system the dimension is from the edge of the obstruction not the center.
Here is some text that might help as well, "Sprinklers shall be permitted to be placed without regard to the blades of ceiling fans less than 60 in. in diameter, provided the plan view of the fan is at least 50 percent open, 8.6.5.2.1.10. This pertains to the blades but not the housing. There is identical text for sidewall, extend coverage and residential sprinklers in 13 systems.
