  I will write only for myself, but I've seen this issue come up on
mailman-hosted lists, too. I use pine as my MUA and it's not a major issue
for me. The default header on the reply is to the sender with the Cc: field
to the mail list. I clean that up before sending. The LyX mail list is the
same way.

  FWIW, the mail lists I manage with majordomo (all low volume, btw), don't
have this problem as far as I know. When folks reply the default is to the
entire list. When a recipient's mailbox is full or the address is no longer
valid, I get a message as the maillist-owner. It doesn't cycle in an endless
loop back to the list.
