If you drop Dev=??, the default of Dev=0 will be used - so for your test adding the "dev=0" is unlikely to make a difference. There can be big differences in ALSA software conversion capabilities between "default" and "front". Some sound interfaces can detect what is plugged and so change according to what is plugged in. I don't think that is what you hear. left / right out of sync - do you mean out of phase (i.e. same audio but no stereo effect - to some music sounds muffled/"dead") .
