COM4? 
Are you sure your modem isn't sharing an interrupt with something else?
If you go into minicom, can you reset the modem?
Can you reset the interrupt to COM1 or COM2?
Can you reset the modem in minicom if it's set to COM1 or COM2?

That ought to keep you busy for a while ;-)
