Because the macro is referring to the name once it is in the
*destination*. In the destination - the address header - the first
name is the name which appears first in that field regardless of
address book settings.
Picture this: In a reply message, the address in the To field did
*not* come from your address book. Which is the first name then? How
should TB know the difference?

No. The macro is for the first name in an email address header. The
fields in the address book are no longer connected to the header once
the message exists. I wouldn't expect it to be any other way myself.
